La recherche sur les caractéristiques comportementales et neurologiques du bégaiement développemental a mis en lumière le rôle important des systèmes sensoriels et moteurs dans le déclenchement et le développement de ce trouble de la fluence. Les personnes qui bégaient diffèrent des locuteurs fluents dans la planification et l’exécution de la parole. Les études récentes sur les symptômes du bégaiement et sur l’imagerie cérébrale ont identifié quelques facteurs contributifs à ce handicap. En plus des déficiences d’apprentissage moteur et comportemental, les recherches ont montré des caractéristiques neurologiques structurelles et fonctionnelles anormales chez les enfants et les adultes qui bégaient dans le réseau cortical et sous-cortical impliqué dans la planification et l’exécution sensori-motrice. On trouve ces différences dans la substance grise corticale et sous-corticale et dans les réseaux de substance blanche responsables de communications intra- et inter-corticales. En outre, on a la preuve que des lésions et des déficiences dans ce réseau neuronal peuvent induire un développement du bégaiement neurogène (acquis) chez des adultes normalement fluents.Dans ce chapitre, nous passons en revue les recherches actuelles sur les bégaiements développemental et acquis, en ciblant celles qui portent sur les systèmes sensoriel et moteur sous-jacents à la planification et à la production de la parole, et nous proposons une conception multifactorielle du début et du développement du bégaiement. Nous dégageons aussi quelques implications cliniques des découvertes de la recherche actuelle.
