Transtorno do Espectro Autista em meninas: uma análise comparativa envolvendo estudos de gênero e possível sub reconhecimento na população feminina

Abstract

Objetivo: A maior prevalência em meninos com Transtorno do Espectro Autista (TEA) sugere modelos comportamentais e vieses masculinos nos atuais instrumentos relevantes para a avaliação diagnóstica. O atual estudo propõe descrever o perfil epidemiológico de pacientes com Transtorno do Espectro Autista (TEA) em crianças de um serviço público de Brasília, dando enfoque a possíveis diferenças de gênero, a fim de incentivar novos trabalhos sobre o tema. Metodologia: Estudo realizado em 2 etapas. A primeira consistiu em uma análise retrospectiva de prontuários dos pacientes e a segunda se deu pela aplicação de questionários direcionados aos responsáveis das crianças, avaliando os critérios do DSM-5 e aplicando os instrumentos diagnósticos ASSQ, ABC e M-CHAT. Resultados: Não foram encontradas diferenças epidemiológicas entre os grupos. Nos critérios do DSM-5 foi observada distribuição diferente dos domínios entre meninos e meninas. No ASSQ, ambos os grupos obtiveram pontuação semelhante e no ABC, as meninas obtiveram maior pontuação. O M-CHAT -CHAT foi desconsiderado para análise, pois havia apenas 1 paciente elegível para aplicação. Conclusões: Percebe-se que existem diferenças no diagnóstico de meninas com TEA, sendo que nas meninas o predomínio dos critérios A e B do DSM-V (comunicação e interação social, padrões restritivos e repetitivos de comportamento), enquanto que nos meninos há uma distribuição mais homogênea na sintomatologia e na gravidade. Mais estudos sobre esse tema ainda muito desconhecido são necessários.
