Abstract
Demans, yaşlanan toplumlarda giderek artan bir halk sağlığı sorunudur. Dünya genelinde 2019 yılında yaklaşık 50 milyon kişi demans ile yaşamakta iken, bu sayının 2050’de 150 milyonu aşması beklenmektedir. Erken tanı, hem yaşam kalitesini korumak hem de bakım maliyetlerini azaltmak açısından önemlidir. Birinci basamak sağlık hizmetleri, bireylerin ilk başvuru noktası olması nedeniyle bu süreçte merkezi rol oynamaktadır. Demans gelişiminde yaş en güçlü risk faktörüdür, ancak hipertansiyon, diyabet, obezite, sigara, fiziksel inaktivite ve işitme kaybı gibi değiştirilebilir riskler de önemli katkıda bulunur. Erken belirtiler unutkanlık, yürütücü işlev bozukluğu, dil güçlükleri ve davranışsal değişikliklerdir. Birinci basamakta Mini Mental Durum Testi (MMSE), Montreal Bilişsel Değerlendirme (MoCA) ve saat çizme testi gibi kısa tarama testleri kullanılabilir. Tanıda ayrıca laboratuvar tetkikleriyle geri dönüşümlü nedenlerin dışlanması gerekir. Erken tanının önünde hekim, hasta-aile ve sağlık sistemi kaynaklı engeller bulunmaktadır. Yönetim; farmakolojik tedavilerin yanında bilişsel stimülasyon, egzersiz, beslenme düzenlemesi ve aile desteğini de içermelidir. Multidisipliner yaklaşım ve toplum temelli farkındalık çalışmaları sürecin başarısını artırır. Sonuç olarak, birinci basamakta erken tanı ve önleyici stratejiler, demans yükünü azaltmada kritik öneme sahiptir.
