Objectif : la prédiction du devenir moteur dans les encéphalopathies anoxo-ischémiques (EAI) est complexe. Nous avons cherché à évaluer l’apport d’un examen utilisant l’Alberta Infant Motor Scale à l’âge de 4 mois (AIMS-4M) dans l’établissement du pronostic moteur à 18 mois. Méthode : nous avons recruté une cohorte rétrospective de patients traités par hypothermie thérapeutique pour une EAI à l’hôpital Sainte Justine (Canada) entre 2009 et 2016. Le suivi neurodéveloppemental était réalisé à 2 et 4 mois en utilisant l’AIMS, et le devenir moteur était classifié à l’âge de 18 mois par le Gross Motor Function Classification System. Une régression ordinale a été réalisée afin de mesurer le pouvoir de prédiction du devenir moteur par le score AIMS et l’IRM cérébrale. Résultats : parmi les 52 nouveau-nés inclus : 16 enfants (31 %) avaient un développement moteur normal, 25 (48 %) présentent un retard de développement moteur, et 11 patients (26 %) ont développé une paralysie cérébrale. Quarante-et-un enfants avaient un score AIMS-4M anormal (inférieur au 10ième percentile). L’IRM précoce et le AIMS-4M étaient tous deux prédictifs d’une motricité altérée (χ2 = 6,1, p = 0,013 et χ2 = 4,9, p = 0,029, respectivement). Un AIMS-4M normal permettait d’exclure une paralysie cérébrale future. Interprétation : l’évaluation par l’association de l’IRM cérébrale et du AIMS-4M permet une prédiction plus précise de la motricité à 18 mois.
