anterior interventricular artery supplying the anterior septum and the anterolateral wall; the circumflex artery supplying the lateral wall arose from a large posterolateral branch with a retrocardiac course.In agreement with the cardiologist and cardiac surgeon, the patient was managed medically with anti-ischemic, antiplatelet, and statin therapy.This is the first reported case of LMCA agenesis being found in a very old patient.The prevalence of this abnormality may be underestimated because it can only be diagnosed using angiography or coronary CT or at postmortem examination.The peculiarity of this case lies in the clinical aspects; the patient's angina pectoris developed only in very old age, whereas the mean age at diagnosis of the cases reported in the literature was approximately 50, and to the knowledge of the authors, the oldest living patient diagnosed with LMCA atresia was a 68-year-old woman.Atherosclerotic plaque is uncommon in LMCA atresia; among the cases reported in the literature, the prevalence of this association was approximately 15%. 3 In true cases of LMCA atresia, the coronary arteries are free of atherosclerotic stenoses or calcifications; this patient's atherosclerotic plaque was only hemodynamically significant in the terminal branches.The presence of calcifications in her right coronary was probably attributable to her cardiovascular risk factors and age.The diagnosis of LMCA atresia was based on the fact that it was impossible to cannulate the left coronary artery that was not visible from sinus in coronary angiography or coronary CT.Although the surgical reconstruction of a two-coronary arterial system is the procedure of choice for pediatric patients (to provide an adequate anterograde flow and make the left-sided myocardium independent of the inadequate collateral flow coming from the right coronary artery 3 ), a coronary artery by-pass might be considered for elderly patients, particularly if they have atherosclerosis, although a medical approach was preferred as the first-line therapy for this patient because it was felt that a coronary artery bypass was unsuitable because of the narrow diameter of her coronary vessels and her old age.
