Fibromyalgia is a rheumatic syndrome characterised by diffuse and chronic muscle pain, and low pain threshold on at least 11 tender points (out of 18 over the body).Other associate symptoms may be sleep disturbance, morning stiffness, fatigue, anxiety, depression, paraesthesia, memory blanks, dizzyness, chest ache, chronic headache, dyspnea etc.While assessing subjects' quality of life, this study aimed at evaluating the discriminating power of instruments used to assess fibromyalgia patients.The sample was made up by 482 subjects divided into Test Group (TG, 302 with fibromyalgia) and Control Group (CG, 180 healthy ones).Only 9 subjects were male: 6 patients and 3 healthy ones.TG subjects, averaging 48,7 ± 10 years old, were outpatients at the Rheumatology Clinic of the Hospital das Clinicas of the University of São Paulo (HCFMUSP); the control group, averaging 49,1 ± 9,1 years old, consisted of healthy volunteers chosen from the hospital staff and from people accompanying patients.Several instruments were used: Fibromyalgic Impact Questionnaire (FIQ) and Medical Outcomes Study 36item Short-Form Health Survey (SF-36), to assess quality of life; STAI for anxiety; PSI for sleep; a ruler for flexibility; Beck's Depression Inventory; and, to assess pain, the McGill Pain Questionnaire, the visual analogic scale (VAS), and an algometer to measure pain threshold on tender points (generating TP index).In order to assess instruments discriminating power, measures obtained were submitted to descriptive analysis through percentile comparison, inferential analysis using the Chi-square Test, and Yule's association.Statistical significance was set at the 0.05 level.All instruments were found to show good discriminating power between the two groups (p<0.05).STAI-trait and tender point algometry show the highest discriminating power -since respective Yule's test values are outside the interval [-0.6;0.6]-, followed by VAS, FIQ, and PSI.Yule's test points to negative association for TP (-0,62), SF-36 (-0,11), and PSI (-0,58), and positive for the other instruments.As to pain level, 75% of GT subjects had over 10 positive tender points and GC ones, 19%; but some values were very high in both groups, like the cervical tender point (90,2% in GT and 68,9% in GC).Findings show that, as compared to healthy ones, subjects with fibromyalgia have higher levels of pain, anxiety, and depression, less body flexibility, and worse sleep and quality of life.All instruments revealed good discriminating power between the two groups, specially the STAI-Trait, algometry, VAS, and FIQ.The high presence of positive tender points among CG allows suggesting that painful points may be a characteristic of Brazilian population.
