Infraslow Electroencephalography Changes in Infantile Spasms (P6.014)

OBJECTIVE: To describe and to correlate with the clinical data the ictal patterns of infraslow (ISA) EEG changes in patients with infantile spasms. BACKGROUND: ISA EEG analysis has shown potential in differentiating absence and partial complex seizures as well as pre-epilepsy surgical planning in adult patients. Despite these potential applications, there have been no published reports, to our knowledge, involving ISA analysis in infantile spasms. DESIGN/METHODS: EEG recordings of all cases of infantile spasms in the past 10 years at the Alberta Children's Hospital were reviewed and for each patient, the first 10 confirmed spasms were analyzed for ISA patterns as follows. Each spasm, was evaluated using an ear referential montage with a band pass filter set to 0.01 Hz to 0.1 Hz. Spasms were then evaluated for correlating changes in ISA and whether these changes, if identified, were either focal or generalized. Once the EEG analysis was completed for all patients, pediatric neurology clinic notes and brain imaging reports were reviewed. Imaging results, treatments, clinical course and information pertinent to likely etiology of the infantile spasms was recorded. RESULTS: 85% of subjects had clear ISA changes correlating with infantile spasms. 46% showed lateralization with at least some spasms analyzed. 100% of patients with generalized ISA changes showed at least a partial response to initial therapy, while this was the case in only 60% of patients with lateralized ISA changes, and 50% of patients with no ISA changes. 33% of patients with generalized ISA changes developed other seizure semiologies, however this occurred in 100% of patients with either lateralized or absent ISA changes. CONCLUSIONS: There are ictal changes in ISA in the majority of infantile spasms. Some trends were observed suggesting that the presence of generalized ISA changes in patients with spasms may confer a better prognosis with regards to the development of other seizure types.
