Introduction: Hospitalization is a necesary treatment in a severe course anorexia nervosa. Nevertheless a significant % of anoretic teenagers can be treated at home, since comorbidity is less frecuent. There is a lot of families willing to try. A clear, detailed treatment plan seems necessary. Hypothesis: Inpatient treatment is necessary in AN. Many AN teenagers can be treated at home. The programme (HCH) is effective. The HCH is cheaper and less traumatic. There are some parents good enough. Objectives: HCC: a ehavioural and familial programme of wich aims are: The recovery of a BMI compatible with outpatient treatment. The introduction of a regular meals pattern. The removal of those social interactions with contribute to the maintenance of the problem. Inclusion criteria: Anorexia nervosa, first episode. EDNOS with rapid weight loss. “Motivated” patients. The patient and her family accept the programme. Parents are being able to cope. BMI more than 14 or –1.5 SD.No suicidal risk. No comorbidity. No cardiological or medical risk. Five steps programme: Cut off mainteinance factors. 0 privilege: complet rest, refeeding I and not bathroom. Refeeding II. Inmediate reinforce (shower, telefone, games, TV). Refeeding III. Inmediate and delayed reinforce (visits, strolls, study) Refeeding IV. Partial return to school. Refeeding V. School and social activities. Progressive refeeding: Increasing refeeding along the programme with increasing caloric intake. Assisted eating: the parents help to eat, to serve dinner, to wait at table, to keep time. Give emotional support during meals and after eating. Working with families: Information to the patient and the parents about the characteristics, significance and consequences of the illness an managment of treatment. Parents organization to cope the ilness. To stay with the patient in order to: help to reffeding; help to rest; help to cope anxiety; to care the ill daughter. To encourage the patient to quit off the illness. Avoid criticism and false atribution. Objectives: Assess “Alexithymia” as a variable in patients with dysthymia. Material and Methods: Alexithymia Toronto Scale 20-items (TAS-20) and Zung`s Self-Rating Depression Scale (SDS) applied to 30 subjects. Mean (X), standard deviation (SD), positive cases percentage (%), Kolmogorov-Smirnov test, t-Student test accepting p ). Depression in dysthymic disorders and alexithymia showed a positive lineal correlation, r=+0.62. Conclusions: Dysthymia shows increased alexithymia.
