Population-level monitoring of STOP HIV/AIDS pilot project activities across Vancouver, British Columbia

Background: STOP HIV/AIDS, a 3 year government-funded pilot project, aims to reduce new HIV infections in British Columbia through expanding HIV-related services. Activities across the city of Vancouver strive to improve early detection of HIV, and ensure timely access to HIV care. To determine if these initiatives are meeting objectives, monitoring of population-level health indicators was conducted. This evaluation has allowed for informed resource allocation, initiation of new programs, and monitoring of targets. Methods: Public health surveillance, HIV laboratory testing and near real-time clinical monitoring were used to assess indicators representing the entire patient journey. Linkages of clinical and public health datasets allowed for population-level monitoring during the pilot project, and comparison to historical trends. In addition, analyses of pertinent sociodemographic, epidemiological and clinical factors provided a more complete picture of the effect of this project for the population of Vancouver Results: Point of care (POC) test volumes at community testing sites accounted for initial increases in testing, followed by increases in HIV tests in clinics located in neighbourhoods with previously known high HIV prevalence. Initially, testing volumes did not change in low risk neighbourhoods, but broad based testing strategies in hospitals and primary care aim to effect change there. Data showed a 40% spike in new HIV diagnoses, with the highest percent positivity among men aged 30-49. Enhanced public health follow-up of partners identified more new HIV positives per test than population-based testing across Vancouver. The proportion of patients currently engaged in treatment increased, with mean community viral load levels steadily declining. However, further analysis revealed the absolute number of potential transmitters (as measured through high viral load levels or not linked to care) in Vancouver had not changed. Conclusion: Preliminary findings provide direction for further strategies to enhance testing and treatment in Vancouver. Areas to focus efforts include certain geographies of concern with low testing rates, sustainment of enhanced public health follow-up and uptake of early treatment to reduce community viral load. In particular, innovative strategies to reduce the significant socioeconomic barriers to adherence in treatment are required.
