Résumé
Sir: I read with interest the editorial by Dr. Nahabedian published in the December 2006 issue of Plastic and Reconstructive Surgery. 1 It has always been a thought in my mind as to what defines plastic surgery as a specialty, especially when teaching undergraduate medical students. Unlike neurosurgery or cardiothoracic surgery, it is not defined by any specific anatomical area, and unlike pediatric surgery, it is not defined by a specific patient population. Plastic surgery is indeed a specialty defined by the techniques used. It tries to restore form and function to those unfortunate patients who have lost it through disease or injury, and in some cases it tries to improve on normalcy and aim for perfection. From the editorial, it appears to be at a disadvantage, since other specialties can “invade” plastic surgery, but it is my feeling that this can well be turned into an advantage. Plastic surgery is not defined by an anatomical area; therefore, plastic surgeons are qualified to operate in areas that are perceived to be the realms of other specialties, as long as their skills are useful there. I remember attending a lecture given by Professor Zuker from Canada in which he talked about his experience as a microsurgeon in helping cardiothoracic surgeons with pediatric coronary artery anastomoses and hepatic surgeons with vascular anastomosis in children. Joseph Murray, who performed the first renal transplant, is indeed a plastic surgeon. As for the cosmetic private practice, which every other specialty wants to have a go at, most patients perceive cosmetic surgery to be within the realms of plastic surgery. I do not think any watertight boundaries are possible with operations that can be performed by many specialties, and as long as these surgeons are suitably trained, they should be able to carry out these operations with safety. The problem, as Dr. Nahabedian has correctly pointed out, is that many of them are not suitably trained and pose a risk to the patients. Rajive Mathew Jose, F.R.C.S.
