We hypothesize that in kidney transplant recipients with DGF, the need for longer periods of dialysis is associated with poorer long term allograft function and an increase in complications. METHODS This was a collaborative chart review including Western University (WU) and the University of Saskatchewan (U of S), of all kidney transplant recipients who developed DGF between 2004 and 2011 (n=83). RESULTS A multivariate regression analysis identified donor age (B=-0.23, p=0.017) and length of time needed on dialysis (TND) post-transplant (B=-0.23, p=0.006) as the most significant predictors of creatinine clearance (CrCl) at one year in DGF patients. These patients were divided into three groups depending on TND (group 1: 7 days or less (n=52), group 2: 8-14 days (n=13), and group 3 (n=18): more than 14 days). CrCl at 30 days (42.4, 33.8, 20.0cc/min; p<0.001), 6 months (50.9, 49.8, 37.8 cc/min;p=0.037), and 1 year (51.2, 47.3, 37.3 cc/min, p=0.035) were all significantly different between the 3 groups. The proportion of patients having one or more complications (wound infection or leakage, reoperation, readmission, or graft loss within less than a year) increased with increasing time that dialysis was needed: 15/52 (28.8%), 7/13 (53.8%), 13/18 (72.2%), (p=0.004).
