F1‐02‐05: Apathy in Alzheimer's disease: Towards an integrated research agenda

A Professional Interest Area (PIA) was established to provide scientific discourse on the topic of neuropsychiatric syndromes (NPS) in neurodegenerative diseases. Apathy is the most common NPS in Alzheimer's disease (AD) and is the focus of this workgroup. A panel of experts of apathy in AD were brought together to discuss the research progress to date and outline a future research agenda. There is considerable consensus in the field that apathy is a reasonably well defined and distinct NPS. Furthermore, several studies which have explored the neurobiological and neurochemical underpinnings of apathy in AD have found generally consistent results. Specifically, the monoaminergic and cholinergic systems have been implicated in apathy. Treatments to date have targeted the cholinergic and, more recently, dopaminergic systems. Literature also suggests the involvement of the frontal circuits. There are currently no animal models of apathy, but many rodent models of depression, under careful scrutiny, may be more closely related to apathy models. While the diagnostic criteria for apathy have been articulated, they have yet to be validated as a treatment target. Overlap with cognitive domains, such as executive dysfunction, and other NPS, as well as subtypes of apathy should be explored from phenomenologic, neurobiologic, functional and neurochemical perspectives. A research agenda that investigates the neurobiological and neurochemical underpinnings of the subdomains may refine and validate current apathy assessment tools, predict success in clinical trials and help valuate drug targets. Future research investigating animal models of apathy should also be considered. The panel of experts will cover the research to date in terms of neurobiology, neurochemistry, biomarkers, animal models, diagnosis, clinical assessment and treatment of apathy in AD. They will also outline a research plan as to where the study of apathy in AD should be heading. With regards to apathy in AD, there are opportunities and areas of research that are ripe for further development. The research agenda outlined by this panel of experts is not only feasible, but promising to field of NPS and AD.
