Bile duct injury after laparoscopic cholecystectomy:resection of the entire extrahepatic biliary tree

A 28-year-old woman with biliary colic underwent an elective laparoscopic cholecystectomy. The operation proceeded smoothly and no intraoperative complications were recognized. The patient returned 7 days later with abdominal discomfort and rising bilirubin and alkaline phosphatase levels. Ultrasound revealed a subhepatic fluid collection. At endoscopic retrograde cholangiopancreatography (ERCP) a complete disruption of the common bile duct was seen communicating with the subhepatic collection. Despite the lack of visualization of the proximal biliary tree, an endoscopic stent was placed. The patient was referred to a hepatobiliary and pancreas referral center where CT revealed no undrained foci of sepsis. The endoscopic stent was coiled up within the small, residual, subhepatic fluid collection. Magnetic resonance cholangiography (MRC) was obtained (A) to visualize the proximal biliary tree. The hepatic duct confluence is not visualized, and mild biliary dilatation of five separate ducts is evident converging on what appears to have been the area of the biliary confluence. This injury is classified as a Bismuth Type 4, or, alternatively, a Strasberg Type E4. After referral and workup, the patient was taken 2 weeks later for operative repair and found to have a complete resection of the biliary tree from the proximal confluence down to its junction with the cephalad portion of the pancreas. The endoscopic stent exited the cut end of the bile duct and was coiled up in the subhepatic space along the porta hepatis. Thermal injury, manifested by necrosis and early stricture formation, was seen at the cut ends of the biliary radicals entering the liver. The devitalized tissue was debrided back to healthy bile ducts. The full extent of the injury was precisely defined. The MRC was extremely accurate in predicting the actual biliary anatomy. Five separate biliary radicals were identified “flush” at the liver surface (A): a separate segment 6 duct (A, a), right posterior sectoral duct (A, b), anterior sectoral duct (A, c), a separate segment 4 branch (A, d), left hepatic duct (A, e). MRC was also extremely accurate in delineating normal, nondilated, complicated hilar anatomy in a separate patient without pathology (B, f segment 6 bile duct, g segment 7 bile duct, h anterior sectoral duct of the right lobe draining segments 5 and 8, I left hepatic duct).
