We are grateful to Drs. Davis and Waters for highlighting the need to critically appraise the content of pediatric quality of life/health-related quality of life (QOL/HRQL) measures for a given application and to appreciate the underlying construct(s) intended for measurement. The scope of our review was extremely broad: to describe measures of HRQL used in pediatrics and in pediatric oncology that may potentially be applicable to acute lymphoblastic leukemia (ALL) and to summarize the literature available through April 2000 in pediatric ALL that applied HRQL measures and quantified economic outcomes ( 1 ) . Indeed, we did not focus on some of the issues raised by Drs. Davis and Waters. Rather, our intention was to draw attention to the substantial progress for both generic and pediatric-specific measures in children with cancer. We argued that, given the availability of adequate measures, HRQL measurements should be included in the design of future trials to better understand the effects of treatment on children with ALL. However, we agree that the development of the field is far from complete, both in pediatric and adult HRQL assessment. The point raised by Drs. Davis and Waters regarding the assumption that absence of problems equals the presence of well-being is an issue for the entire field of HRQL measurement and is not limited to pediatrics. Similarly, the distinction between QOL and HRQL is not unique to pediatric measures, nor is there clear consensus on which domains should constitute QOL and HRQL measures. Moreover, distinguishing among functioning, health status, health-related life, and quality of life can be more misleading than useful due to the overlapping nature of the constructs. To expand upon the closing comment by Drs. Davis and Waters that existing instruments should be “critically appraised to ensure that they measure well-being,” potential users should carefully consider whether the available instruments measure the construct(s) in which they are interested, and, if not, should contemplate the development of a new measure if it can be sufficiently justified.
