Understanding contextual factors influencing adoption of a comprehensive school health program

Background Early life and adolescence represents a key stage in life where important opportunities for health are established. This makes it an important stage for public health interventions aimed at improving health in all stages of life. Given the burden of childhood obesity and the fact that the system is confronted with an increasing number of children with chronic medical conditions, it has become even more critical to pay attention to school-aged children. Study findings will provide insight within an intervention scene that is critical for reaching children of varied socio-economic background. Methods Using a case study approach, this qualitative inquiry, was designed to explore and understand factors that influence the adoption of a school health program (Go Girls!). Using multiple cases, the focus is an in-depth exploration of perspectives of the decision-making process surrounding the program adoption. Analysis of participants’ perspectives as expressed through semi-structured interviews and document analysis will provide holistic understanding of the process. Roger’s Diffusion of Innovation theory guides the design and analysis of data. Four schools within the Lakehead Public School Board where the program have been offered and adopted were invited to participate in this study. Key informants within the schools are chosen in order to capture the perspectives from various hierarchical levels. Preliminary Results A key factor for most cases is the fit of the program within the schools’ values and goals. That is, the program appropriately complements the schools’ set goal and vision. Another emerging factor across the cases is the availability of resources for the program. The program is particularly appealing as the schools have to do very little; program delivery (including resources) is provided by a community agency. Conclusions The compatibility of the program to the schools’ values proves to be crucial in their decision to adopt the program. With the increasing priority to address healthy-active living among school-aged children, public health practitioners should consider the complexity of proposed school health programs and ensure that initiatives do not require additional resources from the school’s already strained pool of resource. Key messages We must look beyond the problem and the interventions when addressing healthy active living among school-aged children, especially in school settings where resource constraint is eminent. Despite the conventional wisdom surrounding the use of evidence-based innovations, the reality is that understanding context remains critical for their advancement and effective implementation.
