Pedometer'S Target Of 7500 Steps/day Improves Health Following An Acute Coronary Syndrome

Pedometers are growing in popularity in order to increase physical activity (PA) and improve health. However, the target in number of daily steps required to increase health after an acute coronary syndrome (ACS) is not known. HYPOTHESIS Based on available data, we hypothesized that during the year following an ACS, subjects who maintained more than 7500 steps/day will show better improvement in waist circumference (WC) and lipid profile than those who did not maintained 7500 steps/day. METHOD A comparative descriptive design was used. In this study, 65 subjects randomly recruited during hospitalization for an ACS in a home-based cardiac rehabilitation program using pedometer and socio-cognitive intervention were included. The average steps/day was measured with blinded pedometer (NL-2000) at baseline and at 3, 6, 9 and 12 months following an ACS. Subjects were classified as into the active group (AG) if they had maintained an average of 7500 steps/day from 3 to 12 months follow-up or as the sedentary group (SG) if they did not maintained this target until 12 months. RESULTS: Thirty subjects were classified in AG, whereas 21 subjects were in SG. At baseline, both groups were similar regarding socio-demographic and health characteristics including cardiovascular risk factors. However, at 12-month follow-up, a significant difference was observed between groups for high-density-lipoproteins cholesterol (HDL-C) (SG 1.0 mmol/L [95% CI 0.9-1.1] vs. AG 1.3 mmol/l [95% CI 1.2-1.4]; p=0.002), triglycerides (SG 1.6 mmol/L [95% CI 1.3-1.9] vs. AG 1.1 mmol/l [95% CI 0.9-1.3]; p=0.002), total cholesterol (TC) to HDL-C ratio (SG 3.6 [95% CI 3.3-3.9] vs. AG 2.9 [95% CI 2.6-3.1]; p=0.001) and WC (SG 103 cm [95% CI 98-109] vs. AG 93 cm [95% CI 88-98]; p=0.005). Furthermore, regarding HDL-C (p=0.001), triglycerides (p=0.005), TC/HDL-C (p=0.004) and WC (p=0.019), the group-months interactions were different. Results were similar after adjusting for age, gender and baseline WC. CONCLUSION Maintenance of 7500 daily steps during the year following an ACS may be a target in order to improve cardiovascular risk factors. Targeting maintenance of 7500 steps/day is a realistic goal for many patients after an ACS and could easily be recommended in clinical practice to maintain or improve cardiovascular health.
