Editor: I read with particular interest the article, “Chaos, Turmoil, and Hysteria: Just Another Night in Triage,” which reviewed the problems encountered in triage. The article highlighted two triage systems, the Emergency Severity Index, a five-level triage system studied in this country, and the e-triage system developed in Canada. We have recently reported our experience with the Soterion Rapid Triage System (SRTS). (J Emerg Med 2006;4:461; U.S. Patent No. 6,786,406 B1.) SRTS is a computerized, algorithm-driven five-level triage acuity system. The use of algorithms has the potential benefit of minimizing the variability of the triage decision-making process. The computerized nature of the system allows for the electronic capture and retrieval of all data acquired during the triage process, facilitating future use of this information in a national triage database. We found that nurses rapidly adapted to the system, requiring as little as 30 minutes training. The inter-rater reliability was extremely high, with 89 percent exact agreement between two nurses in the triage level assigned (weighted kappa = 0.87). We also demonstrated the system's validity in its ability to differentiate among different levels of acuity as measured by admission rates, ED lengths of stay, and hospital charges. Finally, we have recently reported our experience with the use of SRTS in children presenting to the ED, which represents the first study in this country on the effectiveness of a five-level triage system in children. Unlike other five-level systems which have reported poor reliability and significant under-triage of children (Emerg Med 2002;14:67; Acad Emerg Med 2002;9:1397; Am J Emerg Med 2005;23[3]:243), SRTS was found to be highly reliable and strongly predictive of clinical outcome and resource utilization. The system's functionality, effectiveness in clinical practice, and availability of electronically archived data are characteristics beneficial to the development of a national standardized five-level triage acuity system.
