Hemodynamic changes during venovenous perfusion-induced systemic hyperthermia in patients with advanced malignant tumor

Objective: To evaluate the hemodynamic changes during extracorporeal venovenous perfusion-induced systemic hyperthermia (VV-PISH) in patients with advanced malignant tumor. Methods: Eighteen ASAⅠ-Ⅱpatients with advanced malignant tumor aged 29-73 years were involved in this study. After general anesthesia, radial artery and right subclavian vein were cannulated for direct MAP and CVP measurement. Swan-Ganz catheter was placed for hemodynamic monitoring.Percutaneous cannulation of both femoral veins for drainage and perfusion allowed extracorporeal flow rate of about 10 ml/(kg·min) through TEMET SYSTEM 1000.After systemic heparinization, VV-PISH began. Hemodynamic parameters were recorded at the different esophageal temperatures during treatment. Norepinephrine or esmolol was used if MAP lowered by more than 30% or HR exceeded 130/min. The amounts of both drugs used were calculated. Results: With the increasing in body temperature, HR, CI, CVP, MPAP, PCWP increased. HR, CI, MPAP, PCWP increased respectively by 55%, 100%, 18% and 49%, and CVP, MAP also changed significantly. At the time of 41℃, 42℃, 42℃1 h, 42℃2 h, 40℃, norepinephrine and esmolol were required. The amounts of both drugs used during 42℃1 h-42℃2 h were much larger than those during 42℃-42℃1 h. With the exception of PCWP which was still high, all the other parameters showed a tendency towards normal at 38℃. Conclusions: VV-PISH induced hemodynamic changes. Norepinephrine and esmolol were required after body temperature reached 41℃. Cardiovascular stability became worse during the period of 42℃1 h-42℃2 h.After the body temperature lowered down to 38℃, PCWP was still higher than normal. The results suggest that special attention should be payed to the selection of patients and appropriate cardiovascular monitoring and treatment are required during VV-PISH.
