The features of central nervous system complication after liver transplantation and its risk factors

Objective To investigate the types, incidence, risk factors and prognosis of central nervous system complication (CNSC) in patients with orthotopic liver transplantation (OLT).Methods 159 patients with OLT were divided into groupⅠ with CNSC and groupⅡ without CNSC, and their clinical parameters were retrospectively analyzed. Results patients of GroupⅠ accounted for 20.1% (32/159) of the whole. The percentages of diffuse encephalopathy (DEP) and central pontine myelinolysis (CPM) were 75% (24/32) and 15.6% (5/32) respectively; The incidence of epilepsy, cerebral hemorrhage and extrapyramidal disorder was similar (3.1%,1/32). In group Ⅰ and Ⅱ before OLT, The percentage of patients who was diagnosed as acute on chronic liver failure (AOCLF), chronic liver failure (CLF) or primary hepatic cancer (PHC) were 50.0%, 21.9%, 28.1%, and 18.9%, 29.9%, 50.4% respectively (P0.01). Patients of Child C accounted for 87.5% and 65.3% respectively (P0.05). MELD scores were 23.30±11.04 in group Ⅰ and 14.01±12.18 in group Ⅱ (P0.05). Total serum bilirubin were 359.8±266.8 μmol/L and 161.0±229.2 μmol/L (P0.01). Serum ammonia were (68.7±25.6) μmol/L and (40.2±15.3) μmol/L respectively (P0.05). During the OLT operation of group Ⅰ and Ⅱ, The volume was of blood loss be (6 208±3 513) mL and (4 412±3 160) mL (P0.05). Patients with the MAP less than 70 mmHg were 93.75% and 66.9%, and the duration of hypotension persisted for (12.3±6.5) h and (7.6±4.7) h respectively (P0.05). After the OLT operation of group Ⅰ and Ⅱ, hospital days in ICU were (6.6±8.6) d and (4.2±12.1) d, and the mortality was 15.6% and 5.5% respectively (P0.05).Conclusion Diverse CNSC may occur after OLT. The risk factors include primary liver disease and its severity before OLT, hemorrhage volume and duration of hypotension during the OLT operation, and types or doses of immunosuppressive regime after OLT. As for the patients with CNSC after OLT, ICU stays and mortality significantly increase.
