Hematopoietic Progenitor Cells and Inflammatory Monocytes Increase in Circulation in Overweight Children

Over 30% of American children are overweight or obese. Obesity is associated with chronic low-grade systemic inflammation; mediated in part by increased circulating inflammatory monocytes derived from hematopoietic stem/progenitor cells (HSPCs). This systemic inflammation may be promoted by expression of Toll-Like Receptors (TLRs) on inflammatory monocytes, and transferred to various body tissues via chemokine-mediated monocyte homing. PURPOSE: The purpose of this study was to determine if the quantity of circulating HSPCs and inflammatory monocytes was altered in obese/overweight children. Additionally, we aimed to quantify the expression of TLR 2/4 and the chemokine receptor CCR2 on inflammatory monocytes. METHODS: 10 obese/overweight (OW) and 12 healthy weight (HW) male children (9-10 years) matched for demographic characteristics were recruited. Peripheral blood was collected in the fasted state for quantification of HSPCs (CD34+/CD38-/CD45+ and CD34+/CD45low), and inflammatory monocytes (CD14+/CD16+/-) with expression of CCR2 or TLR 2/4 by flow cytometry. RESULTS: There was a trend for an increased quantity of circulating CD34+/CD45low HSPCs in OW children (p=0.06); but no difference was seen in more primitive HSPCs (CD34+/CD38-/CD45low). Classical (CD14+/CD16-) and intermediate (CD14+/CD16+) inflammatory monocytes were both increased in OW (p=0.02 and p=0.001, respectively). Circulating CD14+/CD16+/CCR2+ and CD14+/CD16-/CCR2+ cells were increased in OW (p = 0.004 and p = 0.04, respectively). The quantity of TLR2 or TLR4 expressing monocytes did not differ, but there was a decrease in the TLR4 surface protein expression on CD14+/CD16+/ TLR4+ (p=0.04) in OW. CONCLUSIONS: Here, we demonstrate that OW children have an increased quantity of HSPCs, and both total circulating inflammatory monocytes, as well as those expressing CCR2, which may be an underlying contributing factor to obesity-associated chronic inflammation. Decreased content of TLR4 on monocytes in OW may be due to enhanced recruitment of monocytes to tissues promoting local inflammation. The increase in inflammatory monocytes may be due to altered differentiation of HSPCs caused by the obese/overweight environment. Funding provided by Center for Health, Aging, and Disease.
