Résumé

Care partner depression in dementia is a multi-factorial phenomenon with different factors influencing the care partner's experience of caring for a person with dementia. Existing research shows that there is a clear relationship between perceived care burden and depression in care partners of people with dementia[1]. The aim of this study is to examine other factors that relate to depression in care partners, especially investigating the relationship between behavioral changes such as emotion detection and empathy for others in patients with Alzheimer's disease (AD) or Parkinson's disease (PD) and depression in their care partners. [1] Etters L, Goodall D, Harrison BE. Caregiver burden among dementia patient caregivers: a review of the literature. J Am Acad Nurse Pract. 2008;20:423-8. Patients diagnosed with probable AD (N=18, 73.7+10 years) and PD (N=11, 69.3+10 years), along with their care partners (64.3+13 years), participated in the current study. All subjects (patients and caregivers) performed an Emotion Evaluation Test (TASIT-EET). In addition, all caregivers completed the Interpersonal Reactivity Index (IRI – measures empathy), Neuropsychiatric Inventory (NPI) and Geriatric Depression Scale (GDS). Pearson one-tailed correlation was carried out to determine the relationship between the patient's emotion detection ability and empathy (IRI) and the relationship between the care partner's depression (GDS) and the patient's emotion detection ability (EET), empathy (IRI) and neuropsychiatric symptoms (NPS). There was a significant positive correlation between perspective taking (IRI subscale) and patients’ performance on the TASIT-EET (r=.36, p<.05). In caregivers, there was a significant positive correlation between their GDS score and the patient's NPI total score (r=.61, p<.001), and a significant negative association between GDS and empathic-concern (IRI-subscale) (r=-.48, p=.006). The results of this study suggest that in AD and PD, difficulty understanding others’ point of view is associated with their inability to accurately detect emotions. Care partners’ reported depressive symptoms are related to a greater number of neuropsychiatric symptoms in patients and patients’ decreased capacity to empathize with others. These behavioural changes (decreased emotion detection and decreased empathy for others) and their impact on care partners’ mood warrant further study as this may lead to interventions that could decrease care partner burden.
