Florid Foreign Body Granulomatous Reaction Mistaken for Infectious Milliary Abscess Formation:A Case Presentation

The finding of microscopic polarizable foreign bodies with granulomatous reaction is not uncommon in the lung at autopsy. The finding most commonly is identified in patients with a history of intravenous drug abuse with the excipients or filler materials commonly originating from crushed oral prescription medications. We present a case of a patient with short gut syndrome and TPN dependence following bowel resection for Gardner syndrome. He had no known history of drug abuse. The patient presented with vague symptoms suggestive of infection and a thoracic CT scan showing “numerous tree in bud nodules” consistent with milliary abscesses. A positive central line culture was obtained, and the patient was treated with IV antibiotics with apparent nonresolution of symptoms. Several weeks following his initial presentation, the patient rapidly decompensated at home and could not be resuscitated. Gross findings at autopsy included numerous granular nodules within the lung parenchyma bilaterally. Histologic examination of the lungs revealed florid interstitial and intravascular crystalline material with foreign body granulomatous reaction and medial thickening of pulmonary arteries consistent with significant pulmonary hypertension. The ancillary studies revealed high serum levels of hydromorphone and fentanyl and no evidence of bacterial infection. The cause of death was ruled as mixed drug intoxication. This case represents an unusual presentation of an unfortunately common cause of death with an interesting clinicoradiopathologic correlation reinforcing drug abuse as an important clinical consideration.
