Bilateral coronary ostial stenosis after a Bentall procedure

A 44-year-old woman was admitted to our hospital for exertional angina with a strikingly positive stress electrocardiogram (see Supplementary data online, Figure S1). Symptoms began about 8 months after Bentall procedure with a valve conduit (St Jude 23 mm) for a degenerated bicuspid aortic valve, associated with aortic root dilation, without coronary artery disease. A 64-slice cardiac computed tomography (CT) showed bilateral severe coronary ostial stenosis, clearly depicted by multiplanar reconstructed images [A and B (arrowhead) right coronary artery (RCA); (arrow) left main artery (LMA); E: RCA; F: left anterior descending coronary artery (LAD); G and H: left circumflex artery (LCX)] and three-dimensional rendering images (C: RCA, D: LMA; see Supplementary data online, MovieS1). There were no signs of coronary atherosclerosis or aortic complications. Preoperative invasive coronary angiography confirmed CT findings (I and J). Surgical revascularization was performed with a left internal mammary graft on the left anterior descending coronary artery and a saphenous vein graft on the left circumflex artery, while the RCA aortic button was excised, reconstructed with a saphenous vein graft, and reimplanted on the aortic conduit. The clinical course was then uneventful and she remains asymptomatic at more than 3-year follow-up. Coronary ostial stenosis is a rare but severe complication of Bentall procedure with an incompletely understood pathogenesis, cardiac CT represents an excellent tool for the non-invasive diagnosis of this condition, with the capability of showing both aortic and coronary complications.
