Résumé
Evidence-based recommendations in terms of the acute management of spinal cord injury (SCI) are well established 1-3. Nevertheless, medical complications, such as pressure ulcers (PU), still occur during the acute phase following an SCI, which signifies that current gold-standard measures in terms of PU prevention 1, 2 remain deficient. Thus, research for the development of new efficient measures of PU prevention in acute SCI management is essential. The evaluation of new potential preventive measures compared to the current gold-standard measure may allow us to improve standard of care. First, we would like to highlight that we do not have any potential conflict of interest with regards to the use of any treatment modality for preventing PU, in contrast with Professors Gefen and Santamaria, who are members of the Scientific and Clinical Advisory Boards of MoInlycke Health Care, respectively. The multi-layer wound dressing investigated in the current study (Mepilex Border Sacrum) is a registered product of Mlnlycke Health Care. Considering that Professors Gefen and Santamaria have obvious conflicts of interests when discussing the use of wound dressings, we would like to underline that objectivity is as important as rigour when analysing the data. It is somehow surprising that Professors Gefen and Santamaria specifically identified one result in our study to support the use of the multi-layer wound dressing (decrease of high-grade PU) while strongly criticising the author for using the multi-layer wound dressing instead of a gel mattress. We would like to highlight that the hospital centre involved in this study is a Level-1 acute care trauma centre specialising in SCI care. The preventive dressing was compared to the gel mattress as one component of a holistic evidence-based approach 1-3. Moreover, it is important to argue that the application of a multi-layer foam dressing in addition to the gel mattress may also potentially affect the bodyweight force transfer pattern, as suggested by Professors Gefen and Santamaria.
