[P1–314]: COMPARISON OF PERFORMANCE IN LANGUAGE TASKS AMONG HEALTHY SUBJECTS, PATIENTS WITH ALZHEIMER'S DISEASE, BEHAVIORAL VARIANT OF FRONTOTEMPORAL DEMENTIA, AND PROGRESSIVE NONFLUENT APHASIA
Notice bibliographique
Résumé
There are few studies with clinical samples from low and middle income countries, focused on language systematic evaluation in dementia patients. Some entities can represent real challenges for proper diagnostic differentiation, because language impairments can be masked by memory deficits or other cognitive impairments. We conducted a transversal study using data from the Dementia Clinic's database at the National Institute of Neurology and Neurosurgery in Mexico City. We included data of 10 healthy subjects (HS), 10 patients with Alzheimer disease (AD); 5 with late AD (LOAD) and 5 with early AD (EOAD), 5 patients with progressive nonfluent aphasia (PNFA) and 5 patients with frontotemporal dementia behavior-variant (bvFTD). We describe and compare the performance profile in language tasks, and overall cognitive score among diagnoses. Neuropsychological scores were taken from production (Northwestern anagrams), reading and repetition tasks, MINT, nouns and verbs denomination, semantic associations, fluency taks, and for global cognition, the Montreal cognitive assessment (MoCA). To classify the subjects as impaired or preserved, we estimated cutoff points 1.5 SD below the HS group, and based on the ten percentile of the same group. For multiple comparisons between diagnostic groups we used variance analysis and a median test. Comparison of the distribution of the variables was done using Fisher's exact test. Patients with PNFA showed the worst performance in MINT (45.2 ± 8.5 SD), nouns and verbs naming tests (7.4 ± 6.4 and 4.4 ± 5.3), followed by EOAD (59 ± 4.0 SD) and LOAD (12.4 ± 6.3, 10.4 ± 15.9) groups, for the first and second task respectively. The most striking difference was founded in construction (p<0.01), reading (p=0.01) and sentence's repetition (p<0.01), among PNFA, FTD and AD groups. The PNFA group had the worst overall cognitive performance measured by MoCA (11.4±4.5), followed by LOAD (18.8±5.11). Our results show the differences in the performance of several language tasks among the groups studied, after using a short language battery. Sentences production, MINT or naming of verbs, are helpfully tests for the dementia differential diagnosis. Larger studies are needed to characterize language profiles of different neurodegenerative dementias in Latin-American populations. Performance percentages of study groups in cognitive language tests. Graphic 1. † p value corresponding to the Fisher exact test. The percentages of affected subjects are shown schematically in the main tests used. The distribution of percentages shows an inverse relation, that is to say the percentages of the center constitute the main number of affected subjects, and the percentages of the periphery represent the subjects with a preserved language performance. (MINT: Multilingual naming test; ND: Nouns denomination; VD: verbs denomination; SWPMT: semantic word picture matching test; SAT: semántica associations test.
