Paradigm change: a new local health plan

Résumé
Health planning in Portugal is rule by a document called “National Health Plan” (NHP) acting as a base of action, allowing different regions of country to draw up a “Local Health Plan” (LHP). The LHP seeks to respond to health needs of the population, diagnosing the health situation, and characterizing the morbidity and mortality of the population. The LHP focuses on the disease and it is necessary to change paradigm, is crucial to stop focusing on the disease and focus on promote health. The letter of Ottawa seeks this through concrete and effective community intervention, setting priorities, making decisions, planning strategies and implementing them in order to achieve better health. In the Public Health Unit Arnaldo Sampaio (USPAS), this change was already been achieved with the drafting of a LHP based on the salutogenic model where the center of action is health and not disease. To ensure a dynamic and all-inclusive process, the LHP was designed as a website (www.plsar.pt), where all the actors can contribute and has been constructed following strategic guidelines: act in a proactive and organized way on the factors protecting health and on risk factors; provide citizens, professionals and the community with skills that enable them to enhance health protection factors and minimize risk factors, manage health and disease in an informed and effective manner and influence the health status of the community and respond to current and future health needs of individuals, families and the community by mobilizing the widest possible range of community actors with a view to more rational and efficient use of existing resources and stimulating the creation of new resources. Since January 2017, 36 entities joined the PLSAR and 14 events were published (preliminary results). This LHP is unique because it aims fundamentally the protective factors, integrating all the health professionals and community, which together contribute to improve the state of the health of the population. Key messages: PLSAR's perspective is to produce health throughout the community by requesting its membership the promotion of activities in the 5 territories: affections, citizenship, food, movement and environment. The paradigm of making plans centered on the disease and the activity of the health services is abandoned.
