Rapidly growing facial nodule in a kidney transplant patient

A 69-year-old patient presented to our dermatology clinic for a rapidly growing cutaneous lesion on the left cheek (Figure 1), which was present for 3 months. He was known for end-stage renal disease, which had led to two kidney transplants at 34 and 62 years of age. He was on tacrolimus, mycophenolate mofetil, and prednisone for maintenance immunosuppression, uninterrupted the past 5 years. Prior to this triple immunotherapy, he had been on azathioprine and sirolimus since the first kidney transplant. The patient’s physical examination revealed a firm, well-circumscribed nodule on the left parotid area, measuring 2 cm in diameter. The cutaneous biopsy reported an ulcerated lesion with small blue atypical cells (Figure 2), strong positivity for cytokeratins, and chromogranin and synaptophysin markers. CK-20 diffusely marked the cytoplasm and displayed a paranuclear dot pattern. A total body positron emission tomography (PET) scan revealed several hypermetabolic lymph nodes both medial and lateral to the left parotid gland. Two weeks after presentation, the skin lesion was surgically excised by a postauricular incisional approach and a superficial parotidectomy with peri- and inter-parotid lymph node sampling was performed as a firm node was palpated in the parotid basin. The final histopathology reported deep extension to the subcutaneous tissue, microsatellite lesions, and extensive lymphovascular invasion. Lesions were present in 5 out of 7 periparotid and interparotid lymph nodes. Following the surgery, the patient underwent appropriate therapy. Initial follow-up revealed that the patient was healing well with no signs of reoccurrence. However, six months later the same lesion recurred over the left zygomatic area.
