Résumé
In the article by Rudy Zimmer, several arguments were made against a market-driven approach for the provision of travel health services in Canada.1 Pharmacies have recently become an important partner in the delivery of this type of service in Canada.2,3 In the last decade, the provision of travel health services in a pharmacy setting has improved access, convenience and engagement with potential travellers. Although a study in 2015 conducted in a single medium sized pharmacy chain in Alberta4 did indicate that pharmacists had a low baseline knowledge and poor confidence in their abilities to deliver a travel health service, this is not universally the case in my experience. In my practice, we can accommodate travellers who are unable to make an appointment before their trip avoiding exposure to unnecessary risks. Because we can adjudicate drug billing in real-time, it allows the traveller to have a greater role in cost versus benefit decision making. The traveller is usually able to receive all medications and vaccinations with one visit eliminating trips to multiple providers. Finally, we have many opportunities to screen and assess potential travellers during their visits to the pharmacy and increase awareness of pre-travel preparedness. To ensure travellers receive optimal care, pharmacy-based travel services require ongoing collaboration and communication with members of the healthcare team and healthcare professionals providing this service need to be competent to provide this service. The Manitoba Travel Health Network provides a structure to support collaboration and allocation of limited resources (i.e. yellow fever vaccine shortage), share clinical expertise and foster communication between provincial travel health sites which include pharmacies. The College of Pharmacists of Manitoba has developed guidelines for the provision of travel services by pharmacists. Some of these guidelines include the requirement to obtain the Certificate of Travel Health through the ISTM and the requirements to collaborate and communicate the results of the consultation with the patient’s family practitioner and when to refer the patient. I do acknowledge the provision of travel services in Manitoba is unique and recognize there are many healthcare professionals in other jurisdictions providing travel services without specialized training. Travel health is a complex specialty and it is imperative any healthcare professional receive additional certifications to practice in this area. Pharmacists with additional travel health training can improve accessibility, convenience and engage more travellers for before their travels and provide a quality service. The Pharmacist Professional group of the ISTM also endorse the sentiments expressed in this letter.
