میزان های باروری در اکثر کشورهای توسعه یافته و برخی از کشورهای در حال توسعه، از جمله ایران، به زیر سطح جایگزین (کمتر از دو فرزند به ازای هر زن) کاهش یافته است. طی دو دهه گذشته، میزان باروری کل در ایران (میانگین تعداد فرزندان به ازای هر زن)، زیر سطح جایگزین باقی مانده است، به طوری که از دو فرزند به ازای هر زن در سال 1379 (1)، به 8/1 در سال 1385 و سپس 6/1 در سال 1395 کاهش یافته است (2 و 3). تداوم میزان زیر سطح جایگزین باروری دغدغه رو به افزایش کشورهای با باروری پایین است، چرا که سالخوردگی جمعیت و کاهش نیروی کار را تسریع می بخشد، و در نتیجه پیامدهای منفی جدی ای برای توسعه اقتصادی کشور خواهد داشت. بدین جهت، سیاست های جمعیتی که موجب کاهش پیامدهای ناگوار اقتصادی اجتماعی ناشی از باروری پایین می شوند مورد توجه فزاینده ای قرار می گیرند.  این سیاست ها شامل اقداماتی است که در پی معکوس کردن کاهش باروری هستند و یا به دنبال سازگاری و انطباق با روندهای پایین باروری هستند. دسته اول این اقدامات که در پی افزایش باروری هستند عبارتند از اقدامات مثبت، مانند افزایش مشوق های اقتصادی و کاهش هزینه های تولد اضافی، و اقدامات منفی شامل محدود کردن دسترسی به روش های کنترل باروری است. با این حال، افزایش باروری از طریق مداخله دولت، دشوارتر و پرهزینه تر از کاهش دادن باروری است، و حداقل اثر را روی افزایش باروری در بسیاری از کشورهای غربی داشته است. استفاده از اقدامات منفی در گذشته نیز منجر به افزایش موقتی باروری همراه با افزایش در بیماری و مرگ و میر مادران ناشی از ختم بارداری های ناخواسته از طریق سقط جنین ناایمن شده است. نمونه اخیر از اقدامات منفی، محدود کردن خدمات دولتی تنظیم خانواده در ایران است. این محدودیت بیشتر دسترسی زنان در طبقات اجتماعی اقتصادی پایین با تعداد زیاد فرزند را به روش های پیشگیری دولتی (یعنی عقیم سازی، IUD و تزریقی) محدود کرد (4)، بدون آنکه کم ترین اثری بر افزایش باروری داشته باشد، زیرا فقط 7% کاهش باروری در کشور مربوط به استفاده از روش های دولتی پیشگیری از بارداری است (5).  با توجه به موفقیت حداقلی سیاست های افزایش باروری، بسیاری از کشورهای دارای باروری پایین تصمیم گرفتند تا خدمات اجتماعی مرتبط با سن، مانند نظام مراقبت بهداشتی، تأمین اجتماعی، نظام بازنشستگی و آموزشی، را با روندهای پایین باروری و ساختار جمعیت سالمند تطبیق دهند. این تطابق با استفاده از اقداماتی چون افزایش سن بازنشستگی، پذیرش مهاجران و تغییر نظام های آموزشی، مالیات و سلامت انجام گرفت. وضعیت کنونی ساختارهای اقتصادی، اجتماعی و سیاسی کشور آمادگی اجرای چنین اقدامات تطبیقی را هنوز ندارد. بنابراین برای دستیابی به یک سیاست جمعیتی مناسب جهت مقابله با تداوم باروری زیر سطح جایگزین در کشور، ضروری است که درک درستی از شکل گیری قصد باروری یک فرد متأهل داشت، چون قصد و نیت باروری به عنوان پیش گویی کننده کلیدی رفتار باروری شناخته می شود. در این شماره از مجله سلامت اجتماعی، عرفانی و همکاران یافته های جدیدی را درباره شناخت قصد باروری زوجین ساکن شهر تهران و دلایل عدم تمایل آنها برای فرزندآوری ارایه می کنند. یافته های این تحقیق تأثیر گرایش به ارزشهای فردگرایانه، مانند اولویت علایق و نیازهای فردی بر فرزندآوری، را نمایش می دهد؛ به طوری که این تحولات فرهنگی بیشتر در بین افراد در طبقات بالای اجتماعی و اقتصادی جامعه دیده می شوند. یعنی افرادی که علی رغم برخورداری از امکانات اقتصادی و اجتماعی لازم برای داشتن فرزند و تربیت و پروش آنها، تمایلی به داشتن فرزند ندارند. علاوه بر یافته های این تحقیق، سایر شواهد و داده ها از جامعه ایران نشان دهنده این واقعیت است که ایران وارد مرحله دوم گذار جمعیت شناختی شده است که تداوم باروری پایین را مربوط به چرخش فرهنگی از ارزش های سنتی خانواده محور به ارزش ها و هنجارهای اخلاقی فرد محور می داند. این تحول فرهنگی نخست در اروپا رخ داد، و سپس به شمال آمریکا گسترش یافت و اکنون به آسیا رسیده است (6). در واقع، هنجارهای فردی در حال ظهور، ارزش کمتری برای ازدواج و واحد خانواده قائل هستند و پذیرای اشکال متفاوتی از خانواده، از جمله تک والدی، زندگی مشترک بدون ازدواج، طلاق و خانواده بدون فرزند هستند. شواهد دیگری از مطالعات اخیر در ایران، ظهور چنین تغییرات فرهنگی از جمله رشد ارزش های مادی گرایانه و فردگرایی (7)، افزایش خانواده های تک فرزند یا بدون فرزند، تاخیر در ازدواج و تشکیل خانواده، افزایش میزان طلاق و ازدواج های در حال فروپاشی، میزان های در حال افزایش سقط جنین (8)، افزایش روابط جنسی پیش از ازدواج و زندگی مشترک بدون ازدواج و نگرش مثبت نسبت به دوستی ها و روابط جنسی پیش از ازدواج را ثبت کرده اند. بنابراین، فرد گرایی، به منزله اولویت دادن به رفاه و ابراز عقاید و خصوصیات خود، انتخاب شریک زندگی خود و شکل مورد دلخواه روابط و تعداد فرزندان، و تصمیم گیری در مورد تشکیل خانواده و فرزند آوری بر اساس علایق فردی، مبنای سطح پایین باروری است. در نتیجه، اگر دولت تصمیم به اتخاذ هرگونه اقدام و سیاستی برای افزایش باروری در کشور داشته باشد، کاملا ضروری است که چرخش فرهنگی اخیر به سمت ارزش ها و هنجارهای فردگرایانه، که تصمیم گیری افراد برای فرزندآوری را تحت تأثیر قرار می دهند، را به حساب آورد. Fertility levels have fallen drastically to a below-replacement level (under two children per woman required to maintain the population) in most developed and some developing countries, including Iran. Over the past two decades, the total fertility rate of Iran (the average number of children per woman) has remained under the replacement level, dropping from 2.0 children per women in 2000 (1) to 1.8 in 2005 and then to 1.6 in 2018 (2,3). The persistent low fertility rate is an increasing concern for countries with low fertility, as it accelerates population ageing and declining labor force, with the prospect of severe consequences for economic development. As a result, increasing attention is being paid to policies to reduce the socioeconomic burdens of low fertility. These policies can be efforts to reverse declining fertility or to adapt to low fertility trends.  The pronatalist efforts include positive measures, such as raising economic incentives and lowering costs of additional births, and negative measures that limit access to fertility control.  However, increasing fertility through government intervention has been far more difficult and costly, than inducing a fertility decline, with minimal effect on raising fertility in many Western countries, and the application of negative measures in the past resulted in a temporary short-lived fertility rise along with a sharp rise in maternal morbidity and mortality, related to unintended pregnancies terminated by unsafe abortions. A recent example of the negative measures is the limitation in the government-funded family planning services in Iran that largely affected women from low socioeconomic status with large number of children (4), with minimal effect on raising fertility, because the publicly-funded contraceptive methods contributed to only 7% of the country’s fertility decline (5). Due to the minimal success of the pronatalist policies, many low-fertility nations decided to adapt their age-related social services, such as health-care, social security and retirement and education systems, to a low-fertility regime and aging population structure by taking measures such as raising retirement age, admitting immigrants and adjusting education, tax and health systems. For different political and economic reasons, Iran is not yet ready to apply any adapting measures.  Therefore, for adequate policy responses to the country’s persistent below-replacement fertility, a thorough understanding of the formation of a person’s fertility intention, which is viewed as a key predictor of fertility behaviour, is essential. In the article, “ Reasons for intending to have no children in Tehran, Iran ”, published in the current issue of Journal of Community Health , Erfani and his colleague showed that after economic and parity-related reasons, “conflict of childbearing with own personal life, plans and interests” and “being worry about the future of their children”standout as the two most important social and cultural reasons for not wanting any (more) children.  The other results of this study showed that these cultural reasons reported largely from those with higher levels of education and income, and having one or no child, suggesting that their low fertility intention is attributed to socio-cultural reasons rather than economic factors. These findings as well as evidence from other Iranian studies, stated below, are clear evidence for the fact that Iran has entered into a Second Demographic Transition, which relates low fertility to cultural shifts from traditional familial values toward individualization of moral norms and values, which occurred first in Europe, spread to North America and now to Asia (6). In fact, the emerging individualistic norms place less value on marriage and the family unit, and embrace alternate forms of family, including single parent, cohabitation, divorce, and childless family. Further evidence from recent Iranian studies have documented the emergence of such cultural changes, including the growth of materialistic values and individualism (7), rising childless or one-child families, delaying marriage and family formation, rising divorce rates and falling marriage rates, growing abortion rates (8), increasing  premarital sex and cohabitations  and positive attitudes toward premarital dating and sexual encounters. Therefore, individualism is the base for low fertility, as individuals give priority to their “well-being and self-expression”, choose their own partners and their desired form of relationship and number of children, and make family and childbearing decisions based on their self-interests. Consequently, if the government decides to employ any pronatalist measures to increase fertility in the country, it is absolutely necessary to bring into its calculation the recent cultural shifts to individualistic values and norms that affect individuals’ childbearing decision-making.
