281. AN OVERVIEW OF DIETARY INTERVENTIONS FOR CHRONIC FATIGUE IN DIAGNOSED RHEUMATOLOGICAL DISEASES: IMPLICATIONS FOR CHRONIC FATIGUE AMONG VASCULITIS PATIENTS

Background: Interventions for chronic fatigue associated with ANCA-associated vasculitis are unsatisfactory. A proposed therapy for chronic fatigue in other autoimmune diseases is manipulating the gut microbiome in order to adapt T-cell and cytokine signaling. One such method involves dietary interventions. In this presentation, we present a preliminary overview of available systematic reviews on the impact of diet on chronic fatigue and related symptoms for patients with diagnosed rheumatological diseases. Methods: A research librarian (LT) developed the search strategies for the review which included interventions related to dietary interventions related to chronic fatigue within an exhaustive list of rheumatological conditions. The search was completed in the following databases: Ovid MEDLINE(R) In-Process & Other Non-Indexed Citations, Ovid MEDLINE(R) Daily and Ovid MEDLINE(R) (1946 to Present), Ovid EMBASE (1974-Current), EBSCO CINAHL Plus with Full-text (1937- Current), and the Cochrane Library (from inception to August 7, 2018). In addition to the databases listed above, Scopus (1960 to Present) was used to identify any additional potentially relevant studies. Reference lists of related studies and previous systematic reviews were also screened for eligibility. No study design, date, or language limits were imposed on the search. However, results were limited to published systematic reviews and meta-analyses. A total of 3036 results were retrieved and 2466 unique results remained after the deduplication process (see Supplementary Material B). Citations were then exported into Covidence for screening eligibility. Results: The search outlined 2432 articles initially, and this was reduced to 132 eligible articles for screening. Ongoing work to screen and review the papers continues, and the final results will be available for the presentation at the Vasculitis and ANCA Workshop. Based on early results there appears to be some potential for vegan and vegetarian diet intervention to reduce fatigue symptoms in patients with rheumatic conditions. In addition, there is limited evidence for other interventions including: weight loss diets and Mediterranean diets, and inconsistent evidence relating to supplementation with vitamin D and omega- 3, 6, 9 complexes.
