INTRODUCTION: Cognitive impairment, including dementia, is a common but a poorly recognized problem among patients undergoing hemodialysis (HD). Dementia is associated with high risk of death, dialysis withdrawal, and hospitalization. Therefore, it is important to diagnose cognitive impairment as early as possible and to intervene. Frailty is also a risk factor for disability and mortality. The aim of this study was to evaluate the prevalence of mild cognitive impairment (MCI) and sarcopenia among the HD patients, and investigate the relationship between MCI and sarcopenia. METHODS: A total of 421 patients (273 men) who were undergoing HD in 9 facilities for at least 3 months were evaluated. Montreal Cognitive Assessment-Japan (MoCA-J) and grip strength were examined. MoCA is well known to be a more sensitive screening instrument for detecting cognitive impairment than Mini-Mental State Examination (MMSE). MCI is defined when MoCA-J score is less than 26 points. Grip strength was measured twice on both sides, each average value was calculated, and higher value was adopted. We defined less than 26 Kg for men and less than 18 Kg for women, respectively as decreased grip strength. We also analyzed the factors related to MCI. RESULTS: The mean age was 69.8 ± 11.2 years and the median of dialysis vintage was 72 months (IQR: 30-150). The number of diabetic patients was 195 (49.1%). The median of MoCA-J points was 25 (IQR: 21 - 27) and MCI was confirmed to 244 (58.0%). The average grip strength was 22.2 ± 8.4 kg, 139 men (51%), 95 females (64%) showed decreased grip strength. The grip strength and the MoCA-J points were positively correlated, and the frequency of MCI was high in the group with low grip strength (p <0.0001). MCI was associated with age (Odds Ratio, 1.06; 95% CI, 1.04-1.10) and low grip strength (Odds Ratio, 2.20; 95% CI, 1.30-3.72) when adjusting age, gender, dialysis vintage, diabetes / cerebrovascular accidents / existence of limb amputation · body mass index · systolic blood pressure · albumin · phosphorus · hemoglobin · C-reactive protein · β2-microglobulin · grip strength as an explanatory variables. CONCLUSIONS: We found a high prevalence of MCI and decreased grip strength in patients undergoing HD. Age and grip strength are associated to MCI. Strengthening muscle power might be useful for maintaining cognitive function among HD patients.
