Résumé
Objective: To evaluate the prognostic value of the different degrees of abnormalities in polygraphic recordings recordings,carried out in neonates with diverse encephalopathies, regarding the neuropsychomotor development and epilepsy in infancy. Methods: Retrospective study, including 205 polygraphics recording from 100 newborns, 49 preterm and 51 full-term, with 26 to 47 weeks of post conceptional age, enrolled between January 2014 and December 2016, with different sort encephalopathies. The main etiologies of the were Sepsis (44%), hypoxic-ischemicencephalopathy (33%) and periventricular hemorrhage (16%). The analysis was carried out in two ways, one considering the last or the single polygraphic recording, and another focusing on the worst or the single recording, all of them obtained during the hospitalization. The neurodevelopment was evaluated at least once, from 6 to 18 months of corrected age, through Alberta Infant Motor Scale or traditional neurologic examination. Epilepsy in the evolution was evaluated up to the age of 18 months. The recordings were classified as normal or included in one of the increasing degrees of abnormality, abnormal I, II, III and IV, by the Neurophysiology team of Ribeiro Preto University Hospital, Brazil. The presence of abnormal sharp and spiky transients, as well as brief ictal/interictal rhythmic discharges (BIRDs), electrographic/electroclinical seizures and status epilepticus were considered. Results: The classification as abnormal IV for the last polygraphy before hospital discharge was predictive of epilepsy by the age of 18 months. That means 9 times more chance to develop epilepsy as compared to normal polygraphy classification. The area under ROC curve was 0.78.he classification of the last polygraphy as abnormal I was predictive of neurodevelopment delay based upon traditional neurologic exam; however the area under ROC curve was 0.65, poor discriminative power. Conclusions: In neonates with diverse encephalopathies, a polygraphic recording classified as abnormal IV before hospital discharge is predictive of epilepsy in infancy. Thus, for newborns with different sorts of encephalopathies, prospective studies are still necessary to detect high risk of neuropsychomotor delay and epilepsy in the outcome.
