Résumé
We thank Dr Al-Abbadi and Dr Aziz for their interest in our article,1 for their thoughtful comments, and for highlighting the limitations of the study. We do point out in our article that the lower sensitivity in our data may be due to the fact that our study is based on a single-slide interpretation and that this setting probably does not truly mimic the actual practice of cytology, where more clinical and radiologic information may be available and where interpretation is usually made on several slides, including various and complimentary preparation types such as Papanicolaou- and modified Giemsa–stained slides, and possibly cell block and/or ancillary testing. The intention of our article certainly was not to discourage the use or performance of fine-needle aspirations (FNAs) in the evaluation of salivary gland lesions, but rather to (1) emphasize that when presented with excellent diagnostic material our participants were able to provide concordant diagnoses, (2) present a review of cytomorphologic characteristics of adenoid cystic carcinoma, and (3) point out that even in particularly difficult cases of adenoid cystic carcinoma where basaloid features are evident and predominate but the stromal component is scant to absent on FNA material, a descriptive diagnosis of salivary gland neoplasm with basaloid features, in the appropriate clinical and imaging context and with clinical symptoms consistent with perineural involvement, can be followed up with appropriate surgical management.We recognize and tend to point out in similar papers from this group2 that our situation, a proficiency-testing environment, is very different from a clinical day-to-day working environment. The fact that cytologists can make the diagnosis when good material is available for evaluation should only encourage the acquisition of and improve the nature of such samples. We also encourage development of standards such as the proposed Milan System for Salivary Gland Cytopathology, which can help to bring coherence to the field and can improve utility of salivary gland FNA cytology and, thus, patient care.
