Reply: The authors thank Mr. Hopkins for his thoughtful and insightful comments. He is quite correct in his statement that the Hospital and Anxiety and Depression Scale (HADS) has not been specifically validated in the inflammatory bowel disease (IBD) patient population. However, in the absence of an anxiety and depression screening tool that has been validated in the IBD patient population, we felt that the HADS was the best choice given that the HADS had been previously validated in a population of medical outpatients. Ideally, the HADS should be validated in the IBD patient population. We agree that the reporting of the abdominal sensation item in the HADS can result in possible overdiagnosis of anxiety and depression in patients with IBD because abdominal symptoms are often experienced independent of underlying anxiety or depression. However, other items in the HADS, such as fear, difficulty relaxing, and feeling slowed down, are not necessarily symptoms that arise primarily because of IBD itself. As with any other medical outpatient, these symptoms are likely to reflect underlying anxiety or depression. It is not our contention that the HADS be used on its own to diagnose anxiety and depression in IBD, and the purpose of our study was not to test it in that manner. However, we would suggest that the HADS be used as a screening tool to identify patients with possible anxiety and depression. The fact that its sensitivity may be higher and its specificity may be lower in the IBD patient population would actually be a positive attribute. Patients who are identified as possibly having these anxiety and depression by virtue of the HADS should be referred to a clinician who is expert in the diagnosis and management of these conditions for formal assessment and treatment.
