To the Editor: It was with a great deal of interest that I waded through the article by Hurwitz et al1.Hurwitz EL Morgenstern H Vassilaki M Chiang L-M Adverse reactions to chiropractic treatment and their effects on satisfaction and clinical outcomes among patients enrolled in the UCLA neck pain study.J Manipulative Physiol Ther. 2004; 27: 16-25Abstract Full Text Full Text PDF PubMed Scopus (88) Google Scholar entitled “Adverse Reactions to Chiropractic Treatment and Their Effects on Satisfaction and Clinical Outcomes Among Patients Enrolled in the UCLA Neck Pain Study.” I recognize that the authors were concentrating on adverse clinical effects; however, I think their article would have been more helpful to clinicians if they had addressed the issues that I raise below. Perhaps these questions are the subject of another paper. Perhaps I missed the answers in the data presented. Why was a control group not included? The control group could have received only heat and/or electrotherapy and/or no treatment other than the management listed in the treatment protocol section. It would be helpful to see the rate of adverse effects occurring irrespective of the intervention. What was the clinical outcome, with and without adverse effects, at each of the time frame indices? The choice of the method of treatment should be based on the expectation that there will be an improvement in clinical outcome because of the treatment. If the reward is an earlier improvement in clinical outcome using a particular treatment (ie, manipulation), patients may wish to receive that treatment even with the knowledge that there is an increased likelihood of adverse reactions. It would be helpful to know which treatment, manipulation or mobilization, had a better clinical outcome at the 2-, 4-, 6-, 13-, and 26-week time frames. However, the comparison is not very relevant, in my opinion, unless compared with a no treatment control group. Thank you for your assistance.
