Résumé
Thank you for your interest in our article. We appreciate your response and have considered and discussed it among ourselves, the authors. While we acknowledge that fluid type may certainly alter the frequency and/or severity of fluid extravasation during hip arthroscopy, we did not feel that the data included in our review were sufficient to draw such a conclusion. Only 56% of the included studies reported using normal saline, and some case reports did include the use of Ringer's lactate. Thus, while there is no doubt that the 2 fluids are physiologically different, it would not have been accurate to comment on fluid type beyond providing descriptive statistics. On the topic of the seriousness of the complication and the potential need for surgical intervention, we believe this was supported by our data. We specifically outlined the fact that some fluid extravasation can occur in normal, asymptomatic patients during hip arthroscopy. The fact remains, however, that all case reports of symptomatic extravasation required aggressive intervention, which at a minimum included intensive care unit admission. Therefore, we stand by our conclusion that while some fluid extravasation is normal, symptomatic fluid extravasation is a serious complication that may require aggressive intervention up to and including surgery. In conclusion, we agree that fluid type, as with many other aspects of hip arthroscopy, should be standardized and better reported. However, to draw a link between type of fluid and the frequency of this rare complication would have been beyond the scope of extant data. Given that both fluid types were reported in cases of symptomatic fluid extravasation, the only assertion we can make with certainty is that the use of one fluid type does not completely eliminate extravasation. Is Fluid Type and Not the Procedure the Culprit in Fluid Extravasation?ArthroscopyVol. 33Issue 8PreviewIt was with some interest that I read the article “Fluid Extravasation in Hip Arthroscopy: A Systematic Review,” by Ekhtiari et al.1 This consisted of a retrospective review of 14 studies involving 22 occurrences of symptomatic hip fluid extravasation. The authors conclude, in a rather alarming manner, “fluid extravasation is a rare but potentially life-threatening complication.” There is also a recommendation to consider surgical intervention.
