VP36.04: Early childhood neurodevelopmental outcome after fetal late onset growth restriction

Late onset intrauterine growth restriction (IUGR) results from placental insufficiency, and it has an adverse impact on brain development. We sought to demonstrate the early childhood neurodevelopmental outcome after fetal late onset growth restriction. 27 late onset normal and 17 late-onset IUGR fetuses were studied through the Obstetric Outpatient Clinic at Mount Sinai Hospital, from December 2013 to December 2015. Developmental follow-ups were taken by neonatologist and developmental pediatrician at Mount Sinai Hospital. At four, eight and twelve months, the infant neurodevelopment in motor, language, social and cognitive skills were evaluated. Bayley scores were collected at 18 and 36 months. Weight, height and head circumference are significantly lower at birth, four months, eight months and twelve months, including lower weight in fetal period and lower head circumference at 18 months (figure 1). The gross motor score by AIMS measured in normal infants at eight and twelve months are significantly lower in the IUGR. Language score and rank are lower in IUGR at 18 months by Bayley III test of infant development. Our follow-up neurodevelopmental testing and measures of head circumference indicate that there may be the potential for catch-up growth and development following late onset IUGR. This final observation is a key consideration for the clinical implications of our results. We have demonstrated a higher incidence of poor neurodevelopmental scores at the early ages among infants who were born following a pregnancy complicated by IUGR. IUGR is an independent variable for poor neurodevelopmental outcome. These patients will be followed to preschool ages for further neurocognitive assessment.
