To the Editor: Dr. Piek and colleagues were among the first to identify evidence linking fallopian tube carcinoma (FTC) to BRCA1 germline mutations through their molecular studies (1), as noted in my review. Their support for the modifications of the current International Federation of Gynecology and Obstetrics staging system for FTCs initially proposed by Alvarado-Cabrero et al. (2) is appreciated. Ovarian and FTCs and a number of carcinomas of various primary sites are linked with BRCA germline mutations. Dr. Piek suggests that the term “female adnexal tumor,” referring to both ovarian and FTCs, should become part of the nomenclature for BRCA-related tumors. Although this suggestion is conceptually acceptable, such terminology is a departure from the accepted designation of tumors by their specific primary organ of origin. Whether primary peritoneal carcinomas detected in the adnexa would be included in this new designation of female adnexal tumor is also unclear. Finally, potential would exist for confusion with the entity “female adnexal tumor of probable Wolffian origin” (FATWO). In Dr. Piek's study of fallopian tubes from prophylactic adnexectomy specimens, a high frequency of tubal dysplasia and hyperplasia was found in both BRCA1-positive and BRCA1-negative women (3). In our experience, the finding of FTC and carcinoma in situ in these cases is restricted to BRCA mutation-positive women (4). Other reports have also noted an association between BRCA mutation positivity and FTCs (5–7). The true frequency of tubal hyperplasia, dysplasia, and carcinoma in prophylactic salpingo-oophorectomy specimens and their relationship to BRCA germline mutation status requires further study. The hypothesis that changes in the expression of cell cycle proteins p21 and p27 precede the loss of normal BRCA1 and p53 function is an intriguing one. As our knowledge of the pathogenesis of FTC expands, new issues and questions, such as these, will need consideration.
