The vast majority (96%) of 383 Swedish children born at 22 or 23 weeks of gestation had at least one diagnosis when they were 2-13 years of age. A national register-based study by Morsing et al,1 covering the period 2007-2018, reported that 75% had neurodevelopmental disorders, such as attention-deficit hyperactivity disorder or autistic spectrum disorder. In addition, 88% had selected somatic disorders, including asthma and cerebral palsy. Just over a half of the children (55%) required habilitation support. Gridneva et al3 describe environmental determinants of human milk composition in relation to health outcomes. Beggs et al4 summarise the evidence from clinical trials on human milk fortification for preterm infants. Meanwhile, Picaud highlights the importance of donor human milk being available for very low-birth weight infants.5 From 2009 to 2018, unsafe sleep environments contributed to, or resulted in, the deaths of 145 infants aged 0–24 months in Manitoba, Canada. Godoy and Maher retrospectively reviewed their autopsy results and medical records and concluded that several risk factors had increased over time. These included bedsharing, objects in the sleep environment and not sleeping on an approved surface, defined as a crib or bassinet.6 Infants were overrepresented in sleep-related deaths if they were Indigenous, came from low-income neighbourhoods or had young mothers. Congenital central hypoventilation syndrome (CCHS), also called Ondine’s curse, is a rare disorder that affects central control of breathing and non-invasive ventilation has been the first-line choice for Swedish patients. However, problems such as unintentional leakage, and midface hypoplasia have been identified with long-term use. Tsolakis et al7 studied 23 patients with CCHS who received diaphragm pacing implants at a mean age of 9.1 years and reports that 20 made the successful transition from non-invasive ventilation. The authors also respond8 to questions from Alibrahim and Esquinas about the strategies for tube removal and how families were helped to adjust to the new technology.9 Marfan syndrome is a multifaceted genetic disorder that involves multiple organ systems, such as the cardiovascular and skeletal systems. An observational study of 130 children with Marfan syndrome by Stark et al10 showed that the presence of a tricuspid valve prolapse increased the likelihood of other cardiac and extracardiac manifestations in these patients. The authors suggest that tricuspid valve prolapses should be routinely and regularly assessed when paediatric Marfan patients undergo echocardiography.
