Résumé
HISTORY: A 66-yr-old women 12 months post- left knee TKA. Chief complaint includes persistent left knee pain to the antero-lateral aspect of the knee joint and lateral aspect of the patella. Pain and limitations reported in both daily living and recreational activities. Patient needs a cane for prolonged walking and hiking and requires bilateral handrail assist to perform stairs. Patient is highly motivated and adherent to physical therapy treatment plan but has reached a therapeutic plateau without any further progress. PHYSICAL EXAMINATION: At orthopedic physical exam, left knee presents full ROM, no instability and negative J-sign. Main findings include persistent quadriceps and hamstrings weakness, Iliotibial band retraction, poor hip stabilization and poor proprioception. DIFFERENTIAL DIAGNOSIS: 1. Patellar maltracking 2. Ilio-tibial band syndrome 3. Chronic synovitis 4. Prosthetic loosening 5. Quadriceps tendonitis TEST AND RESULTS: Left knee radiographs: • Normal lower limb, patellar and prosthetic alignment • No sign of prosthetic loosening Dynamic echography: • Mild articular swelling • Signs of chronic synovitis • Signs of antero-lateral soft-tissue restriction at the PF compartment possibly linked with scar tissue Knee Kinesiography (dynamic 3D functional assessment): • Neutral frontal alignment maintained dynamically • Excessive external tibial rotation during swing to heel strike phase suggesting altered patellar tracking • Limited flexion excursion (absorption) during loading phase known to increase PF joint loads FINAL WORKING DIAGNOSIS: 1. Dynamic patellar maltracking 2. Chronic synovitis TREATMENT AND OUTCOMES: 1. Home based biomechanical corrective and neuromuscular focused exercise program aiming to change push-off/swing pattern addressing external tibial rotation movement 2. Education to manage chronic pain and inflammation 3. Prescription of topical anti-inflammatory medication, twice daily. At 4-month follow-up, Knee Kinesiography retest showed restored optimal push-off pattern without the presence of excessive external tibial rotation. Patient reports walking 2 hrs /day without pain and inflammation, has resumed short hikes, climbs stairs without handrail assist (descent with single handrail assist), is able to play doubles tennis with limited effusion.
