Gender Characteristics of Postoperative Cognitive Dysfunction in Patients Undergoing Coronary Artery Bypass Grafting

Введение. Послеоперационная когнитивная дисфункция (ПОКД) является частым осложнением коронарного шунтирования (КШ), характеризующимся нарушением памяти и способности к обучению, снижением внимания и скорости обработки информации. На сегодняшний день влияние фактора пола на развитие ПОКД после операции КШ практически не изучалось. Цель. Изучить гендерные особенности развития ПОКД в раннем послеоперационном периоде у пациентов, перенесших КШ. Материалы и методы. В проспективное когортное исследование включены 50 мужчин и 31 женщина в возрасте от 45 до 70 лет, поступившие на плановое КШ. Все пациенты проходили предоперационное клинико-инструментальное обследование и двухэтапную оценку когнитивных функций: нейропсихологический скрининг с использованием Монреальской шкалы когнитивной дисфункции и расширенное нейропсихологическое тестирование показателей исполнительных и психомоторных функций, внимания и кратковременной памяти, на 8–11-е сутки после КШ повторно проводилось расширенное тестирование с определением ПОКД согласно критерию «20%-20%». Статистическую обработку данных проводили по программе STATISTICA 10 (StatSoft, Inc., США). Результаты. Установлено, что пациенты женского пола чаще страдают от развития ПОКД, чем мужчины (74% vs 56%), отношение шансов (OR) составило 2,24, 95% ДИ (CI) 1,23–4,06, p=0,01. В структуре ПОКД у мужчин наблюдаются нарушения функций всех когнитивных доменов: психомоторных и исполнительных функций, внимания и кратковременной памяти, тогда как у женщин чаще отмечаются комбинации нарушений памяти с вниманием или исполнительными функциями. Бинарная логистическая регрессия позволила выявить ассоциации стенозов сонных артерий, сахарного диабета 2-го типа и высокого уровня тревожности с риском развития ПОКД, тогда как пол пациентов был менее значим, χ2=20,06; p=0,001. Заключение. В настоящем исследовании выявлены гендерные особенности в развитии ПОКД в раннем послеоперационном периоде у пациентов, перенесших КШ. Пациенты женского пола чаще страдают от развития ПОКД, чем мужчины. У мужчин чаще наблюдаются нарушения во всех когнитивных доменах, тогда как у женщин чаще отмечаются комбинации нарушений памяти с вниманием или исполнительными функциями. Более значимыми независимыми факторами риска развития ПОКД, чем пол пациентов, были наличие стенозов сонных артерий, сахарного диабета 2-го типа и высокий уровень тревожности. Introduction. Postoperative cognitive dysfunction (POCD) is a common complication of coronary artery bypass grafting (CABG) characterized by impaired memory and learning ability, decreased attention and speed of information processing. To date, the effect of gender on the development of POCD after CABG has not been studied. Purpose. To study the gender characteristics of POCD in the early postoperative period in patients undergoing CABG. Materials and methods. The prospective cohort study included 50 men and 31 women aged between 45 and 70 admitted for elective CABG. All patients underwent a preoperative clinical and instrumental examination and a two- stage cognitive functions assessment: neuropsychological screening using the Montreal Cognitive Assessment scale and extended neuropsychological testing of indicators of executive and psychomotor functions, attention and short-term memory, extended testing was repeated at 8–11 days after CABG with the definition of POCD according to the criterion «20%-20%». Statistical data processing was performed using the STATISTICA 10.0 (StatSoft, Inc., USA). Results. Female patients were found to be more likely to develop POCD than males (74% vs 56%), with an odds ratio (OR) of 2.24, p=0.01. In the POCD structure in men, impaired functions of all cognitive domains were observed: psychomotor and executive functions, attention, and short-term memory, whereas in women combinations of memory disorders with attention or executive functions are more frequently observed. Binary logistic regression revealed associations of carotid artery stenosis, type 2 diabetes mellitus, and high levels of anxiety with the POCD risk, whereas patient gender had a lesser significance, χ2=20.06; p=0.001. Conclusion. This study revealed gender characteristics in the development of POCD in the early postoperative period in CABG patients. Female patients are more likely to develop POCD than males. Male patients are more likely to experience cognitive decline in all domains, while women are more likely to experience combinations of memory impairment with attention or executive functions. The presence of carotid stenosis, type 2 diabetes mellitus and a high level of anxiety were the more significant independent risk factors for the POCD development than patient gender.
