This editorial heralds 2022, a new year filled with hope for all of us. We are anticipating a status change from a COVID pandemic to an endemic prevalence. The policy implications remain vague. For the first time in Canada, Mental Health and Addictions is represented in the Federal Cabinet at the ministerial level. A good omen! Best wishes, Dr. Bennett. This issue reflects the fact that the Canadian Journal of Addiction also welcomes international submissions deemed to be of interest to our practice of Addiction Medicine. The report by O’Keefe-Mackman, Vojtila, and Fischer raises awareness about the recent changes in drug preference from opioids to psychostimulants. Psychostimulant use, including co-use with opioids, results in enhanced physical and mental morbidity as well as elevated risk of contracting human immunodeficiency virus and hepatitis C virus. Tailored interventions, as compared to those targeting opioid use, remain scarce, perhaps due to the lack of approved pharmacotherapies. Marques-Pinto, Machado, and Dias-Amaral, of Oporto, Portugal, contribute a systematic review of the role of drugs of abuse as both trigger and maintaining factors for sexual dysfunction, affecting all phases of sexual response models. Failure to address these drug induced dysfunctions precipitate relapses and poor relationship functioning. A PLISSIT model (permission, limited information, specific suggestions, intensive therapy) is proposed. We just had the third anniversary of the legalization of cannabis. Time to review our experience. This issue contributes to the database with two articles. Rittenbach et al linking Alberta's administrative data from 2012 to 2018 compares health utilization rates, mental health comorbidities, and demographic characteristics. While cannabis use is highest among individuals under 18 and 18 to 25 year olds, the largest increase is found to be among the 55 years and older group. Pham et al compares data from the 2017 Canadian Tobacco, Alcohol and Drugs Survey (CTADS) with the 2019 post-legalization Canadian Tobacco and Nicotine Survey. In this early comparison, the prevalence of Canadian cannabis use postlegalization increased by approximately 2%. Next, the Geneva group analyzes early, unplanned 30 day readmissions to a hospital addiction unit. Penzenstadler et al's study of 240 consecutive SUD admissions identifies a 10% increase in the Brief Symptoms Checklist score at discharge as best associated with a 4.2% increase in early readmissions. The findings add to the need to pay attention to the presence of comorbid disorders. From a centralized medication monitoring database (PharmaNet) in BC covering 5657 mostly family physicians, Klimas et al estimates that over one year (December 2018 to November 2019), the main monthly initiation rate of opioid analgesics to opioid naive patients was 2.05 prescriptions per physician. Lastly, this issue concludes with a thought provoking case of a patient in sustained remission from opioid use disorder following a year of treatment, who faces deportation due to criminal behavior conducted while in active and untreated addiction. The discussion by Jadir and Gill related to this case provides a useful window on a rarely reported facet of stigma in our field. We hope that the diversity of this issue will be of help.
