Tooth replacement by means of dental implants has been widely adopted. In many clinical situations, it is the preferred option, or the sole solution, to replace missing teeth with a fixed implant supported prothesis. Over four decades, dental implant therapy has evolved significantly and the current state of knowledge is supported by substantial scientific evidence. We have gained understanding of all phases of implant dentistry and implant behavior over years of clinical use. Like in other interventions in healthcare, there are associated complications, development of peri-implant disease being one of the most significant ones in implant dentsitry. This special issue was designed to compile the current state of knowledge on peri-implantitis, with a comprehensive approach that focuses on the physiopathology, predisposing factors, preventions strategies, surgical and chemotherapy treatment approaches, and the role of the abutment/prosthetic design in the development of this disease. The reader will have the opportunity to understand the mechanisms of the peri-implant disease and how the peri-implant tissues are affected by it. The latest information on the regenerative techniques for both soft and hard peri-implant tissues is provided in depth. There is also a broad assessment on peri-implantitis as a multifactorial condition triggered or influenced not only by microorganisms but by an array of factors such as implant type, implant position, peri-implant phenotype, abutment configuration, prosthetic design, occlusion, and all systemic and habitual patient-related factors. The ultimate scope of this compilation of original articles presented by select leaders in the field is to serve the dental profession with an across-the-board analysis of the current state of knowledge on peri-implantitis. Deep understanding of the disease and its pluri-factorial nature could help attune the dental profession to minimizing the incidence of peri-implantitis. By applying treatment strategies and protocols guided by a treatment planning philosophy that encompasses all of the aspects discussed in this special issue, we will be undertaking the initial step toward the prevention of peri-implant diseases. My deepest gratitude to all the authors and co-authors for their dedication to the profession and for sharing their wealth of knowledge for the betterment of our patients.
