Changes in Frailty Index After Emergency Surgical Interventions in Elderly Patients with Acute Abdominal Pathologies

Введение. Острые абдоминальные патологии у пациентов пожилого и старческого возраста имеют свои специфические свойства, которые непосредственно влияют на результаты лечения. Цель. Изучить динамику изменений индекса хрупкости до и после операции у пожилых пациентов с острыми абдоминальными патологиями. Материалы и методы. Проанализированы результаты хирургического лечения 255 пожилых пациентов с острыми патологиями органов брюшной полости. У всех пациентов вычисляли индекс хрупкости в день госпитализации, через 7 и 30 дней после операции с помощью опросника Эдмонтона. Выжившие пациенты были приглашены в стационар для повторного обследования через 1 год после операции. Наряду с общими обследованиями изучалось динамическое изменение индекса хрупкости. Результаты. Индекс хрупкости в день поступления был равен 8,39±0,15. При детальном анализе было установлено, что у 118 пациентов (46,3%) индекс хрупкости находился в пределах от 7 до 9, у 69 (27,1%) – от 9 до 11; у 34 – ≥11 (13,3%), а у 34 (13,3%) – ≥12. Через 30 дней после операции отмечено снижение индекса до 7,27±0,14, разница была статистически значимой (р<0,001). Сравнительный анализ между подгруппами выявил, что ни у одного из пациентов при поступлении не было индекса хрупкости от 5 до 7 баллов, в то время как через 30 дней после операции у 69 пациентов (27,1%) – от 5 до 7, что считается склонностью к хрупкости. Количество лиц с индексом хрупкости от 7 до 9 уменьшилось со 118 до 82 (p<0,0001). У пациентов с индексом хрупкости до 9 он достоверно снизился в течение 30 дней после оперативного вмешательства. В течение года после операции наблюдался рост числа пациентов с индексом хрупкости >12. Заключение. Определение индекса хрупкости у пациентов пожилого возраста при поступлении в стационар, а также динамическое наблюдение в разные сроки после операции может быть полезным при оценке интенсивности реабилитационных процессов. У пожилых пациентов с индексом хрупкости от 7 до 9 результаты, полученные в ближайшем и отдаленном периодах после операции, были более обнадеживающими. Лица с индексом хрупкости от 9 до 12 должны находиться под динамическим амбулаторным наблюдением. Introduction. Acute abdominal pathologies in senile and elderly patients have their own specific properties that directly affect the results of treatment. Purpose. To study the dynamics of changes in the frailty index before and after surgery in elderly patients with acute abdominal pathologies. Materials and methods. The results of surgical treatment of 255 elderly patients with acute pathologies of the abdominal organs were analyzed. In all patients, the frailty index was calculated on the day of hospitalization, 7 and 30 days after surgery using the Edmonton questionnaire. Surviving patients were invited to the hospital for re-examination 1 year after surgery. Along with general examinations, the dynamic change in the brittleness index was studied. Results. The frailty index on the day of admission was 8.39±0.15. In a detailed analysis, it was found that in 118 patients (46.3%) the frailty index was in the range from 7 to 9; in 69 (27.1%) from 9 to 11; in 34 people ≥11 (13.3%); and in 34 patients (13.3%) ≥12. 30 days after surgery, frailty index decreased to 7.27±0.14, the difference was statistically significant (p<0.001). Comparative analysis between subgroups revealed that while none of the patients had a frailty index of 5 to 7 at admission, 30 days after surgery, 69 patients (27.1%) had an index of 5 to 7, which is considered to be frailty prone. The number of persons with frailty index from 7 to 9 decreased from 118 to 82 (p<0.0001). In patients with a frailty index of up to 9, the frailty index significantly decreased 30 days after surgery. During the year after the operation, there was an increase in the number of patients with a frailty index >12. Conclusion. Determination of the frailty index in elderly patients upon admission to a hospital, as well as dynamic observation at different times after surgery, can be useful in assessing the intensity of rehabilitation processes. In elderly patients with a frailty index of 7 to 9, the results obtained in the immediate and long-term periods after surgery were more encouraging. Persons with a frailty index of 9 to 12 should be under dynamic outpatient observation with high caution.
