P102 Dissecting the metabolic impact of faecal microbiota transplantation (FMT) in C. difficile and E. coli infections using 3D OrbiSIMS

Introduction Faecal microbial transplantation (FMT) is an effective treatment strategy for patients with recurrent C. difficile infection rCDI. However, its mechanistic actions are not fully understood. FMT also attenuates E. coli infected outgrowth by modulating the intestinal microbiome in mice. This is the first study to utilise novel Orbital Trapping Secondary Ion Mass Spectrometry (OrbiSIMS) to characterise faecal metabolic profiles of patients with rCDI that successfully responded to FMT. We also conducted an integrated analysis of the E. coli metabolome and lipidome to investigate the metabolic effects of co-culturing faecal supernatants from human FMT donors with E. coli. Methods OrbiSIMS was used for metabolomic analysis of faecal samples from rCDI patients (n=12; mean age 63.8 years, female 67%). Faecal samples were collected before FMT (baseline) and after 12 weeks (post-FMT). We evaluated the effects of FMT on the metabolomes of two different pathogenic Enteroaggregative E. coli strains (1×109 CFU of O104:H4 and O20:H19) co-cultured with FMT-donor faecal supernatant samples (n=4) for 4 hours at 37 °C. Results OrbiSIMS metabolomics detected 8362 features. We identified 223 fatty acids, 53 glycerophospholipids, 22 bile acids, 5 sulfatides, and 15 amino acids. Following successful FMT, 15 glycophosphoinositols (GPIs) were significantly increased (p < 0.05) compared to baseline. Post-FMT samples showed a significant increase in histidine and lysine biosynthesis compared to pre-FMT samples. Notably, E. coli metabolome analysis showed higher GPI levels in pathogenic EAEC co-cultured with FMT supernatants compared with FMT donor supernatant samples. This demonstrates that the increase in GPIs levels was microbially derived. Our results suggest that the gut microbiota regulates GPI metabolism. Conclusion In rCDI patients, a response to FMT associates with significant increases in faecal GPIs as well as histidine and lysine biosynthesis. FMT similarly increased abundance of GPI levels in the bacterial metabolomes of pathogenic EAEC strains suggesting that GPIs could represent a potential novel therapeutic target for E. coli and CDI infections. Future studies will utilise OrbiSIMS to profile FMT-related alterations in the bacterial metabolomes of clinically relevant toxigenic strains of C. difficile.
