Objectives Depression is a significant comorbidity in pediatric patients with chronic diseases, including Juvenile Idiopathic Arthritis (JIA).1 This literature review aims to investigate the prevalence, risk factors comprehensively, and outcomes of depression and anxiety disorders in JIA patients, shedding light on the need for mental health screening and support in pediatric rheumatology care. Methods A systematic search was conducted using Proquest (BNI, MEDLINE databases), with search terms ‘Juvenile Idiopathic Arthritis’ AND ‘mental health’ in the title and abstract. A total of 43 studies were initially identified, and through title and abstract screening, 18 studies remained. After a full-text review, 4 studies were included in this review. Results The review encompassed studies from different countries, providing a multifaceted perspective on the mental health of JIA patients. The key findings are as follows: Prevalence and Risk Factors: JIA patients exhibit a higher prevalence of depressive symptoms, clinically diagnosed depression, and suicidal ideation than the general pediatric population[2]. Risk factors include chronic pain, functional limitations, medication use, and the burden of frequent medical examinations. A significant portion of patients, often newly diagnosed, experience clinical depression, emphasizing the importance of early detection and intervention.2 Psychiatric Disorders in Incident JIA Patients: A cohort study in Finland identified a higher frequency of psychiatric diagnoses, especially among female JIA patients.3 Age at onset of JIA appeared to influence the risk of psychiatric illnesses, suggesting potential windows for intervention. Parental Socioeconomic Status: Danish national registers revealed that children with JIA had a 17% higher risk of psychiatric diagnoses, particularly depression and adjustment disorders, compared to their peers. This association was not influenced by parental socioeconomic status.4 Adolescents in Canada: A study in a Canadian sample of adolescents with JIA reported high rates of symptoms of major depressive disorder and panic disorder, highlighting the pressing need for mental health screening protocols and services.5 Conclusion This literature review underscores the substantial prevalence of depression and anxiety in JIA patients, necessitating a shift toward routine screening for psychiatric comorbidities in pediatric rheumatology care. There is an urgent need for standardized assessment tools and a systematic pathway to refer affected children to mental health professionals. Mental health screening should be incorporated into initial pediatric rheumatology assessments and periodic reviews, particularly during periods of increased disease activity. This comprehensive understanding of the mental health burden in JIA patients advocates for a holistic approach to their care, addressing both somatic and psychological well-being, ultimately enhancing their overall quality of life.
