EP16.27: Acceptability and tolerability of magnetic resonance angiography as compared to Doppler ultrasound in the third trimester of pregnancy

Doppler ultrasonography has several limitations for the evaluation of fetal cerebral vasculature. Magnetic Resonance Angiography (MRA), through Time of Flight (TOF) and Susceptibility-Weighted Imaging (SWI), is an interesting new technology allowing visualization of the cerebrovascular system without a contrast agent. The purpose of this study was to evaluate the acceptability and tolerability of fetal MRA in pregnancy and compare it to traditional ultrasound. This study was performed within the context of a pilot study exploring the feasibility of visualization of fetal cerebral vasculature. 10 monofetal pregnancies between 36 and 40 weeks gestational age were recruited. The participants underwent a fetal MRA, as well as a fetal cerebral Doppler ultrasound, to visualise the fetal cerebral vasculature. Prior to each imaging session, participants completed the State-Trait Anxiety Inventory (STAI). Immediately following each examination, comfort and tolerability questionnaires were completed. Out of the 10 MRA conducted, 5 were analysed and most intracerebral main vessels were visualised. The circle of Willis was fully visualised in 3 fetal MRA. Participants expressed minimal anxiety regarding both examinations. However, MRA was found to statistically cause more anxiety, with a median STAI score of 24.50 [20.75-29.00], compared to ultrasound's score of 21.00 [20.00-22.00] (p = 0.042). General discomfort, limited lying position and noises were the main complaints regarding the tolerability of MRA in comparison to ultrasound. All participants demonstrated a high level of acceptance for both imaging modalities, with a global MRA acceptance of 88% [76.50-100]. 100% of participants would agree to undergo MRA again for medical purposes. MRA imaging and Doppler ultrasound allow the assessment of fetal brain vasculature and appear to be well tolerated and acceptable for third trimester pregnant women, this represents a useful information for future clinical research.
