Mind–Body Connection: Mitigating the Harm of Stress through Mindfulness

Dear Readers, Mindfulness represents the “quality or state of being conscious or aware of something.” In today’s world, mindfulness is used in several therapeutic interventions, including mindfulness-based cognitive behavior therapy, mindfulness-based stress reduction, and mindfulness meditation. Educational institutions, workplaces, and even clinical settings have been increasingly recognizing the benefits of mindfulness. This special issue highlights the stressors of cardiovascular disease (CVD) and all the techniques of mindfulness, including yoga, meditation, Tai Chi (TC), in mitigating the harms of stress. Arthur J. Siegel, an experienced primary care physician, contributed an article titled “Icarus Redux: An Octogenarian Primary Care Physician’s Toolkit to Avert Burning Out.” He reflects on the unprecedented burnout rate among physicians in the post-COVID-19 era and suggests that each medical student practice mindfulness from the outset to oppose the anticipated professional stress. In addition, he provides his own toolkit to avert such crisis – starting with a daily affirmation based on inspirational messages from exemplars. The review “Impact of Chronic Psychological Stress on Cardiovascular Disease Risk: A Narrative Review” by Liam Z. Munir and Eugene Francois du Toit summarizes the findings of a comprehensive search of peer-reviewed journals in PubMed/Medline, Embase, Cochrane Library, and PsycINFO. The findings indicate that physiological and behavioral risk factors of psychological stress have adverse effects on cardiovascular health. For example, vagal dysfunction caused by chronic stress is associated with vascular inflammation; stress-induced changes to eating habits, and physical activity levels could increase the risk of CVD. The authors propose that clinicians should acknowledge the adverse effects of chronic stress in individuals with (or susceptible to) CVD and recommend further research to elucidate the mechanisms involved and provide effective strategies to minimize the stress. The review written by Woods et al. describes takotsubo cardiomyopathy (TCM) as a transient and reversible cardiomyopathy and a rare condition. The diagnosis of this cardiac abnormality has increased since its initial description in 1990. On initial presentation, TCM often mimics acute coronary syndrome but may be differentiated with coronary angiography. In addition, transthoracic echocardiography can help with the diagnosis and identification of complications such as left ventricular (LV) outflow obstruction and LV thrombus. TCM continues to be an active area of research as there is still much to be learned regarding its pathogenesis, diagnosis, and treatment. A systematic article and meta-analysis by Mansueto Gomes Neto et al. describes the potential benefits of yoga in the elderly. The authors searched MEDLINE/PubMed, Cochrane, EMBASE, and Physiotherapy Evidence Database from the earliest date available to July 2023. Their conclusion generated with evidence is that yoga could reduce depression, perceived stress, and improved major cardiovascular risk factors in people with coronary artery disease (CAD). Therefore, they propose that yoga be considered as a part of the care of patients with CAD. The article “Effects of Tai Chi and Qigong on Heart Rate Variability: A Systematic Review and Meta-Analysis” by Larkey et al. aimed to synthesize existing literature on the effects of two related forms of meditative movements, TC and Qigong (QG), on heart rate variability (HRV) in adults. Four databases (MEDLINE, Google Scholar, Academic Search Premier, and PubMed) were searched for the articles through December 15, 2023. The authors concluded that TC/QG interventions may shift HRV parameters toward improved health status and resilience. In addition, they also offered suggestions to improve the design and measurement of future TC/QG research interventions exploring HRV. A study titled “Posttraumatic Growth after Spontaneous Coronary Artery Dissection: A Mixed-Methods Study” by Rogerson et al. investigated the nature, prevalence, and correlates of posttraumatic growth after spontaneous coronary artery dissection (SCAD). They used a mixed-methods approach that included two parts, and demonstrated that despite the associated stresses and challenges, there is great potential for positive growth after SCAD. These findings have implications for the development and targeting of interventions to support post-SCAD recovery. The authors, from various parts of the world, acknowledge the potential of mindfulness in offering a more balanced, resilient, and healthier society that fosters well-being and compassion.
