Background Laboratory medicine is a significant global healthcare activity.[1] Clinical laboratories have a crucial role in delivering patient care by ensuring the quality of medical laboratory testing to aid clinical decision-making.[2] In fact, clinical laboratories provide healthcare professionals with the factual information needed to deliver high-quality, safe, effective, and suitable care for disease prevention, diagnosis, treatment, and management.[3] The first International Conference on Diagnostic and Laboratory Medicine was held in Saudi Arabia, a country renowned for its leadership in the field of healthcare excellence. The conference was designed to explore the evolving landscape of diagnostic and laboratory medicine. It offered an opportunity to review recent findings, new technologies, and effective approaches to improve diagnostic performance, laboratory efficiency, and patient care. At the end of the conference, the “Declaration from the First International Conference on Diagnostic and Laboratory Medicine” was passed to provide strategic direction on the future of diagnostics in global health care. Some recommendations were to expand leadership and management and improve hematopathology, toxicology, and histopathology to increase treatment effectiveness for patients. First International Conference on Diagnostic and Laboratory Medicine This paper presents the first International Conference on Diagnostic and Laboratory Medicine that took place in Jeddah, Kingdom of Saudi Arabia, from May 21, 2024 to May 24, 2024. This important event was held with the assistance of the Executive Council of the Makkah Health Cluster, designed to improve the understanding of each other and to promote cooperation in the field of diagnostics quality. This conference was organized by the Saudi Society for Laboratory Medicine in collaboration with various distinguished scientific societies and organizations. The conference featured keynote speakers from some of the most esteemed universities in the United States, Canada, Italy, the United Arab Emirates, and Saudi Arabia. In this event, medical laboratories were highlighted as an essential component of the healthcare system, providing a critical role in the timely detection and management of diseases this importance was evident throughout the conference, focusing on the critical role that laboratories play in enhancing health outcomes. The event was divided into preconference workshops and main conference sessions spanning 3 days. The preconference workshops addressed topics such as leadership and management in medical laboratories, forensic toxicology, histopathology, hematology, and hematopathology. The main conference on May 22, 2024, consisted of eight sessions covering a range of critical topics in diagnostic and laboratory medicine: leadership and management, genomics and personalized medicine, current trends in clinical microbiology and immunology, health and well-being of pilgrims during Hajj, and quality and productivity in laboratory medicine. Every session offered lessons, and recommendations, focusing on international trends and national contexts. Another key theme of the conference was its relevance to the Saudi Vision 2030, especially the health transformation program. This program addresses the critical need to improve access, quality, and efficiency of healthcare services The conference assessed the development and improvement of policies for effective and sustainable financing of the health sector, as well as in the promotion of healthcare services availability, quality, and preparedness for evolving health risks. Recommendations Leadership and management Encourage continuous learning and growth in strategic planning within the laboratory setting Use key performance indicators to monitor and improve laboratory operations Emphasis on implementing leadership practices such as effective communication, and workload distribution to help employees cope with stress and enhance overall performance Encourage a strong focus on professionalism among laboratory practitioners. This fosters adherence to ethical standards, ensures test accuracy and precision, upholds safety protocols, respects patient confidentiality, and promotes overall integrity and accountability Review and update job descriptions regularly to highlight areas for improvement, comparing strengths and weaknesses Address how to overcome leadership challenges on employee motivation, communication, and engagement, as engaged employees are 21% more productive and 22% more profitable under effective leadership[4] Emphasis on achieving a balance between cost containment and high-quality diagnostic services requires implementing effective resource management strategies Encourage integration of artificial intelligence (AI) and advanced diagnostic technologies into laboratory operations. Toxicology and forensic sciences Encourage universities to establish specialized academic programs in toxicology Develop and strengthen relations among all relevant agencies, including the Ministry of Health, Ministry of Interior, Food and Drug Administration, poison centers, and similar organizations. Histopathology and hematology Introduction of new educational programs to develop and enhance training in hematopathology to effectively meet the increased demand for well-trained hematopathology physicians and technicians In hematology laboratories, coagulation testing can be complex and require a high degree of attention to detail. For this reason, we recommend using interpretation and expert comments on the results from special coagulation testing to ensure accurate results that help clinicians make informed decisions. We also recommend establishing a reference laboratory and a robust referral system for all complex cases to ensure appropriate diagnosis and management Pathologists often face the challenge of effectively collaborating on complex cases. Implement an electronic referral system to reduce communication and decision-making blockages, improving diagnostic accuracy and patient care Set clear local guidelines and protocols for the optimal use of skin biopsies in diagnosis. Hematopathology Use the umbrella category approach for testicular diffuse large B cell lymphoma (DLBCL), recognizing its classification similarities with primary central nervous system DLBCL according to WHO guidelines Differentiate large/high-grade BCL with 11q aberration and Burkitt lymphoma as different entities Carry out a clear distinction between BCL2-R double-hit lymphomas and BCL6-R cases in diagnosis and therapeutic approach Use excision biopsies to diagnose “nodal T follicular helper cell lymphoma, follicular type” (nTFHL-F) and consider using the generic term “Nodal T Follicular Helper Cell Lymphoma” (nTFHL) when interpreting core biopsies to avoid classification error Develop advanced diagnostic tools for the early detection and management of angioimmunoblastic T-cell lymphoma. Artificial intelligence and technology Form a collaborative task force with lab professionals, data scientists, information technology experts, and healthcare executives to drive responsible AI adoption in laboratory medicine Develop internal policies for ethical AI use, emphasizing transparency, accountability, and bias mitigation Bridge the gap between data and practice by integrating AI tools with electronic health records for improved clinical decision-making Invest in robust data infrastructure to handle large data volumes and ensure efficient data storage, retrieval, and management systems for AI applications. Personalized medicine and genetics Acknowledge the importance of addressing residual cardiovascular risk for a thorough cardiovascular disease (CVD) management approach Consider testing on patients with high unexplained low-density lipoprotein cholesterol levels or a family history of CVD to further characterize genetic predispositions affecting Lp(a) levels Integrate immunological profiling in the diagnosis and treatment of breast cancer for a better understanding of disease biology and to guide targeted therapies specifically in cases of BRCA1 and BRCA2 mutations Allow the integration of tandem mass spectrometry with next-generation sequencing technology in neonatal screening procedures Adopt higher-level genomic techniques to increase accuracy decrease false-positive cases and try to achieve a quicker turn-around time (TAT) of screening results. Clinical microbiology and immunology Periodically update clinical guidelines for the most recent innovations in diagnosing and treating autoimmune diseases as well as their implementation in practice Address the major problems of immunodiagnosis in autoimmune diseases. Enhance the level of diagnostic test accuracy and reliability, as well as improve strategies in the screening, confirmation, and monitoring of autoimmune conditions Utilize the different diagnostic techniques – Reverse transcription polymerase chain reaction, nonstructural protein 1 antigen testing, serology, and sero detection methods – to properly diagnose infection by the dengue virus. Population health management and Hajj well-being Dengue fever remains a critical local public health threat. Developing more advanced early detection tools is promptly needed to meet the challenges of dengue fever, improve patient outcomes, and strengthen regional public health responses Enhance laboratory medicine services by focusing on disease prevention and early detection strategies, particularly for diseases like malaria during high-risk periods such as the Hajj season Empower public health laboratories for large-scale disease screening and surveillance, fostering collaboration with healthcare providers and continuous training for laboratory professionals to address both communicable and noncommunicable threats. Quality and laboratory optimization Emphasize the importance of reducing errors in the laboratory to ensure high-quality lab services Closely monitor point-of-care services performed outside the laboratory by nonlaboratory staff to ensure compliance with accreditation standards and accurate bedside testing Utilize auto verification to decrease postanalytical errors, with special rules reviewed by the project team Implement lean thinking and Lean Six Sigma projects to minimize waste, reduce TAT, and manage budgets effectively. Conclusion The first International Conference on Diagnostic and Laboratory Medicine in Jeddah, Saudi Arabia, aimed at advancing medical laboratory sciences. Key recommendations included improving the quality and efficiency of diagnostic procedures, emphasizing continuous education and training for laboratory professionals, and fostering collaboration between healthcare institutions to enhance patient outcomes and disease management. The conference underscored the evolving role of laboratory professionals and the need for innovative approaches to meet modern laboratory requirements. Financial support and sponsorship None.
