The influence of integrative group psychotherapy on the personality and psychological characteristics of patients with somatoform disorders

Somatoform disorders (SFD) are a group of common psychiatric disorders. Psychotherapy is one of the leading methods of their treatment. However, data on its effectiveness in SFD are still ambiguous. The authors developed their own program of integrative group psychotherapy, the targets of which are the basic personality and psychological factors of the SFD development — alexithymia and somatosensory amplification. Purpose of the study: to evaluate the efficacy of integrative group SFD psychotherapy, focused on the correction of basic personality and psychological factors of this pathology, based on the assessment of the dynamics of personality and psychological characteristics of patients during the psychotherapy course. Materials and methods. The study involved 65 patients with SFD, who were divided into two groups: the study group (34 patients) and the controls (31 patients). The patients of the study group received group psychotherapy according to the program proposed by the authors, while the patients of the control group received therapy provided for SFD by the official standards of medical care. The course of treatment in both groups was 30 days. The patients’ condition was assessed using the Toronto Alexithymia Scale, the Sheehan Anxiety Scale, and the Bekhterev Institute Personality Questionnaire. The patients of the study group were additionally examined 12 months after the end of the course. Results. During the therapy period, the patients of the study group demonstrated a significant decrease in the levels of alexithymia (by 13.2 %) and anxiety (by 48.7 %); the structure of personal reactions to the disease changed positively. The degree of most of these changes was statistically significantly higher (p < 0.05) than in the control group. A catamnestic examination 12 months after the end of the course of psychotherapy showed the persistence of the achieved effect in all indicators, and for a number of them (outwardly oriented thinking as a component of alexithymia, the level of anxiety, a reduction in the neurasthenic type of reaction to the disease) — the presence of delayed positive changes. These results may serve as a basis for further implementation of the proposed psychotherapeutic program in psychiatric practice.
